- Description: This synopsis for commercial supplies and services is prepared in accordance with the format in FAR Part 12.6 as supplemented with additional information included in this notice and Simplified Acquisition Procedures in Part 13. This notice constitutes the only solicitation; a Request for Quotation is being requested and written solicitation will not be issued. The NAICS code is 492110 and the size standard is 1500 employees. DESCRIPTION OF REQUIREMENT: The requirement is to furnish all labor, tools, parts, materials, and personnel necessary for courier service for 59th Medical Support Squadron. PERIOD OF PERFORMANCE: Basic Year plus four (4) option years. STATEMENT OF WORK: STATEMENT OF WORK Courier Service for Wilford Hall Medical Center 1. DESCRIPTION: Purpose is to establish a 5-year Blanket Purchase Agreement for courier service for all activities supported by WHMC, starting 1 Aug 2004 and end 31 July 2009. The Contractor shall furnish all labor, vehicles, and equipment necessary to accomplish routine courier service. The 59th Medical Support Squadron will provide the points of pickup and drop-off along the route. The scheduled courier route includes the locations and points of contact provided. The courier service will perform: pick up and deliveries to area hospitals and other facilities in the Greater San Antonio area. 2. STARTING LOCATION OF SERVICE: 59th Medical Support Squadron. 3. SCOPE OF PROJECT: 1. The government reserves the right to initiate bilateral modifications to accommodate future changes to the BPA. 2. A log of record pickup and drop-off will be maintained. 3. Contractor shall provide color differentiated, weather proof containers to transport medical records between facilities. 4. For outpatient records, a pull list (log) has to be signed and updated by the contractor and kept by facility. 5. The contractor has to be willing and able to add at least four more locations (8 stops) in the future. 5. JOB VERIFICATION: 59th Medical Support Squadron personnel will furnish the contractor with any special instructions for pick-ups or deliveries. 6. HANDLING: Contractor shall be responsible for damage/loss during transport and delivery of items. 7. HOURS OF WORK: Normal work hours are 0730 to 1700, Monday through Friday, with the exception of Camp Bullis, which closes at 1600. The following provisions and clauses apply: 52.252-2, Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. The provision FAR 52.212-2, Evaluation-Commercial Items, is applicable to this solicitation and the blanks in section (a) are completed as follows: Price considerations. Offerors shall include with their quotes, a completed copy of 52.212-3, Offeror Representations and Certifications--Commercial Items. Far 52.212-4, Contract Terms and Conditions--Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercials Items; 52.232-33, Payment by Electronic Funds Transfer--Central Contractor Registration, and DFARS 252.204-7004, Required Central Contractor Registration, are applicable to this acquisition.
