Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W91QF504T0008 is the reference number for this Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisiti on Circular 2001-24. This solicitation has been set-aside 100% for small business concerns. The NAICS Code is 561730. Item Description: Contractor shall furnish all labor, supervision and equipment to grind all debris from Hurricane Isabel. The location o f the current debris is at 40th Street and H Ave, Fort Lee, VA 23801. Debris for the purpose of this solicitation is defined as trees, tree stumps, limbs, branches, shrubbery, and any other miscellaneous yard waste. All contractor equipment is to remain a t the job site until job is completed The chips are to remain at the site until job is completed. The mulch pile shall not exceed 10 feet in height and shall be spread uniformly over the entire grinding area. Contractor shall start work within 30 days of n otice of award and shall complete the work within 30 days after the start date. There is a possibility for the debris to increase up to 25% from the date of the site visit to the completion of work. Work can be performed Monday through Saturday between the hours of 7:00 AM and 9:00 PM. However, if any contractor employee works more than 40 hours per week, the contractor is obligated to pay overtime at no additional cost to the government. There shall be a mandatory site visit on July 29, 2004 at 9:00 AM. Al l interested contractors shall meet for the site visit at: Directorate of Contracting, 1830 Quartermaster Road, Building 7124, Fort Lee, VA 23801-1606. POC is Randy Saunders. All correspondence will be in writing. No phone inquiries will be accepted. The following FARS are applicable to this order: 52.212-1, Instructions to Offerors; 52.212-2, Evaluation of Commercial Items- Items (Past Performance, Ability to Meet Constraints and Price); 52.212-3- Offeror Representations and Certifications- Commer cial Items; 52.212-4, Contract Terms and Conditions- Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items. All quotes are due no later than 4:00 PM on Friday August 6, 2004 and shoul d submit with their offer the following on past performance: Commercial business references from 3 companies with whom you maintain a business working relationship and are currently or have done business with in the last three (3) years. Each reference sho uld consist of a complete mailing address, a point of contact (POC), a telephone number and a fax number for the POC, a brief description of the type of relationship or contract. This business should be able to provide information on their business experie nces with your company relating to a. history, b. customer satisfaction. Also, a statement to the effect that the work will begin 30 days after notice of award and all work can be completed within 30 days of start date.
