- This is a combined synopsis/solicitation for a commercial item prepared in accordance with the Federal Acquisition Regulation (FAR) Part 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 22. This Procurement is issued as a 100% small business set-aside under SIC Code 5169 with a size standard of 100 employees. The agency need is to provide X-ray film processing drain cleaner and services for Navy Public Works Center San Diego in accordance with the attached specifications. Payment will be made via Government-Wide Commercial Purchase Card. X Ray Film Processing Drain Cleaner and Services. The NPWC SD has a requirement to clean/maintain 8 X-Ray processor Drains per month at the Navy Regional Medical Center, San Diego Specification for X-Ray Processor Drain Cleaner 1. Must be in ?Kit? Form. One kit cleans/maintains one drain. 2. An industrial drain opener/maintainer designed specifically to break apart the crystalline buildup in the first 4 feet of an x-ray processor drain. 3. An industrial drain opener/maintainer designed specifically to break up the black sludge buildup in the first 20 feet of the x-ray processor drain. 4. Designed to identify any dry traps and faulty plumbing that could allow dangerous sewer gases and foul odors to enter the facility. It is also an industrial strength deodorizer. 5. Contractor will schedule training on the proper use, handling and disposal of the products within the kit. The contractor trains PWC maintenance personnel on the correct application of the product. Assist PWC personnel in application of the product for the first two months. Follow up weekly with PWC personnel to help troubleshoot any problems that may arise in the x-ray processor drain area. 6. Representative shall be available throughout the year to schedule on-going training and safety seminars upon request. 7. Guarantee product that after four (4) weeks of application, the drains will not clog. If that happens, the contractor will cover the cost to unclog 8. If there is damage to equipment, facility or personnel due to the application of the product in accordance with the label, the contractor will cover the cost. Contractor is required to break down pricing by CLIN as follows: CLIN 0001 Base Year: Kit Unit Price $______ x 8 EA x 12 Months = CLIN 0002 Base Year: Kit Unit Price $______ x 8 EA x 12 Months = CLIN 0003 Bas e Year: Kit Unit Price $______ x 8 EA x 12 Months = CLIN 0004 Base Year: Kit Unit Price $______ x 8 EA x 12 Months = CLIN 0005 Base Year: Kit Unit Price $______ x 8 EA x 12 Months = The requirement is for a firm-fixed price type contract. Contractor is required to provide a technical plan explaining how they will meet the technical specifications (type of kit cleaner to be used, including technical specifications) and also a statement on its delivery schedule. In accordance with 52.211-8 Time of Delivery, all services must be performed in accordance with the schedule?no deviations are allowed. All prospective offerors are cautioned that in the event the Government is unable to determine price reasonableness based on the offers received and market analysis, the Government will not issue a contract. This decision is solely the decision and right of the Government. All e fforts performed and all cost expended by prospective offerors in providing information requested by the solicitation (including potential site visits, if requested) are not subject to reimbursement by the Government. The following provisions apply: FAR 52.212-1, Instructions to Offerors Commercial Items, Addendum to FAR 52.212-1, Paragraph (b) (5) Offers shall provide an express warranty which at a minimum shall be the same warranty terms, including offers of extended warranties, offered to the general public. DFAR 252.204-7004 Required Central Contractor Registration, applies to all solicitations issued on or after 6/1/98; Lack of registration in the CCR database will make an offeror/quoter ineligible for award. The Government intends to make a single award to the responsible Offeror whose offer is the most advantageous to the Government considering price and price related factors. The Government intends to award the contract to the eligible, low-priced, responsible offeror. Provision 52.212-2 Evaluation Commercial Items applies with paragraph (a) completed as follows: Award will be made to the Offeror that meets the solicitation?s minimum criteria for technical acceptability at the lowest price. To be determined technically acceptable the Offeror must furnish product literature that demonstrates that the offeror can meet all requirements stated in the above purchase descriptions. All responsible Offerors are to submit current pricing, delivery time, payment terms, cage code, Dunn and Bradstreet number, Tax Identification Number and all applicable specifications regarding this solicitation. Also required are copies of applicable commercially published price lists pertaining to your company?s services that meet the specifications, along with applicable government discounts.
