Description

The Social Security Administration (SSA?s) office at 1710 Gwynn Oak Avenue, Baltimore MD 21207 was flooded on July 7, 2004 and has been closed and inaccessible to SSA since then. Any documents sent to the fax machine at that location ((410) 966-9310) on or after July 7, 2004 are lost. Any documents delivered to 1710 Gwynn Oak Avenue on or after July 7, 2004 may have been lost. Quotations sent via email have not been lost and do not have to be resent. SSA-RFQ-04-0498 is amended as follows. 1) The item description is clarified as follows: SSA requires that the hampers be priced and delivered fully assembled. Pricing quoted must be for delivery of fully-assembled hampers. 2) The response date is changed from: 3:00 p.m. Eastern Daylight Time, July 8, 2004 to: 3:00 p.m. Eastern Daylight Time, July 23, 2004. You may fax quotations to: (410) 966-0870 or email to: andrew.klimas@ssa.gov. SSA will consider for award all quotations received to date by the SSA contracting officer. If there is any question whether SSA has received your quotation, please resubmit it by the revised closing date and time stated above
