- DESCRIPTION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request for quotes are being requested and a written solicitation will not be issued. This procurement is for a firm fixed price contract. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. The North American Industry Classification System (NAICS) is 424720 with a size standard of 100 employees. This solicitation is not set-aside for small business. I. SPECIFICATIONS: Supply, deliver, (FOB destination) and apply an estimated 440 tons of anionic high float polymer modified emulsion (HFE-90-IS). Delivery is to Forest Service Road 28 at Elder Pit (T34, R17, Sec 32). From Klamath Falls just east of Bly turn north on County Road 1259, approximately 1 mile to Forest Service Road 34. Proceed on Forest Service Road 34 to Forest Service Road 28, travel north on Road 28 to Elder Pit. PROVISIONS AND CLAUSES: The following provisions and clauses are hereby incorporated into this RFQ by reference, (full text of the clauses may be accessed electronically at www.arnet.gov/far/): FAR 52.212-1, Instructions to Offerors-Commercial Items; FAR 52.212-2, Evaluation Commercial Items (see below for evaluation factors); FAR 52.212-3, Offerors Representations and Certifications-Commercial Items; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; FAR 52-212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items [in paragraph b check blocks 1, 14, 15, 16, 17, 18, 19, 20, 25, and 30]. FAR 52.211-6, Brand Name or Equal. No addenda apply to these provisions and clauses. III. QUOTING REQUIREMENTS: Vendors wishing to respond to this solicitation shall provide this office with the following: a price quote on company letterhead for the requested items showing the unit price for the supply, delivery and application of the chip seal oil. The quoter shall also include a delivery date. Delivery shall be coordinated with Roland Glade, Forest Road Maintenance Manager. The quoter shall also provide a pamphlet about the product offered showing specifications with the quote. Quoters shall provide a listing of contracts completed in the last 3 years of similar work (include project name/identification, contract name and telephone number). Quoters shall also include a completed copy of provision FAR 52.212-3, Offeror Representations and Certifications Commercial Items, with their quote. IV. AWARD: The Government will award contract(s) to the responsible offeror(s) whose offer conforming to the solicitation represents the best value to the Government, price and other factors considered. The following factors will be used to evaluate quotations: (1) experience, (2) time of delivery, (3) past performance (customer satisfaction, business relations) and (4) price. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) database prior to award. See Clauses 52.204-7 Central Contractor Registration (OCT 2003). The Government may reject any or all quotes, issue orders to other than the lowest quote, and waive minor informalities or irregularities in quotes received. If necessary, the Government may conduct discussions with any quoter. V. QUOTE DUE DATE: Quotes shall be received by July 30 at 4:00 p.m. Only written quotes (mail, fax copy or hand carried quotes) will be accepted. Mail or hand carry quotes to the following address: USDA Forest Service, Fremont-Winema National Forests, Acquisition Management, 2819 Dahlia Street, Klamath Falls, OR 97601. Fax number is (541) 883-6709. Quoters shall verify Government receipt of fax copy quotes prior to closing by calling Lavon Ostrick at (541) 883-6804. All quotes should be clearly marked with the quoter???s name and solicitation number R6-20-04-16DB.
