- Description
- (i)This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) RFQ FA5270-04-T-0016 is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as a request for quote (RFQ). (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Regulation Circular 2001-23. (iv) This acquisition is unrestricted. (v) The requirement is for the operation and maintenance of the Personnel Alerting System (PAS). The scope of work for the PAS includes but not limited to new installation of equipment, removal, repair, relocation, upgrades, and Preventive Maintenance on Kadena Air Base, Okinawa Japan. A detailed statement of work is provided in Attachment 2, entitled Statement of Work. (vii) The contract performance period is 1 Oct 04 ? 30 Sept 05, with four (4) one year option periods. (viii) The provisions of FAR Clause 52.212-1 Instructions to Offerors -Commercial Items applies to this acquisition. (ix) The provision of FAR Clause 52.212-2 Evaluation-Commercial Items is applicable. See Attachment 1 for specific evaluation criteria. (x) Offerors must include a completed copy of the provision at FAR 52.212-3 Offeror Representations and Certifications?Commercial Items, with quote. See Attachment 1. (xi) The clause 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. (xii) The clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items, applies to this acquisition. Specifically the following clauses cited are applicable to this solicitation: FAR 52.203-6 Restrictions on Subcontractor Sales to the Government; FAR 52.225-13 Restrictions on Certain Foreign Purchases; FAR 52.232-34 Payment by Electronic Funds Transfer?Other Than Central Contractor Registration. (xiii) The following provisions and clauses apply to this acquisition: FAR 52-225.14 Inconsistency between English Version and Translation of Contract; FAR 52.232-18 Availability of Funds; FAR 52.233-2 Service of Protest; FAR 52.217-8 Option to Extend Services; FAR 52.217-9 Option to Extend the Term of the Contract; FAR 52.245-1 Property Records; 52.245-2 Government Property; The clause 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders ? Commercial Items, applies to this acquisition. Specifically the following clauses cited are applicable to this solicitation: FAR 52.203-3 Gratuities; DFARS 252.205-7000 Provision of Information to Cooperative Agreement Holders; DFARS 252.225-7012 Preference for Certain Domestic Commodities; DFARS 252.243-7002 Certification of Requests for Equitable Adjustment. Additional clauses included in the RFQ are DFAR 252.225-7041 Correspondence in English; DFARS 252.233-7001 Choice of Laws; DFARS 252.245-7001 Reports of Government Property; AFFARS 5352.242-9000 Contractor Access to Air Force Installations. The above referenced clauses and provisions applicable to this acquisition shall be listed in Attachment 1 with a bid schedule. Attachment 4 contains Air Force Occupational Safety and Health Standard 91-50 A site visit will be conducted on 15 July 2004, at 0800 Japan Standard Time. Past Performance Questionnaires (attachment 3) must be completed and returned as outlined in Attachment 1, not later than 29 July 2004, at 4:00 P.M., Japan Standard Time. Please forward your desire to attend the site visit via email to samantha.block@kadena.af.mil no later than close of business on 8 July 2004. The Government will award a firm-fixed price contract resulting from the combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government. Best value can include but is not limited to price and past performance. Submit written offers only, oral offers will not be accepted. NOTE: This contract will be performed in its entirety in the country of Japan. Therefore, award can only be made to firms licensed and authorized to perform work in the country of Japan. (xvi) All quotes must be emailed to Samantha Block at samantha.block@kadena.af.mil, or hand carried to 18 Contracting, Bldg 732, and Attention to Samantha Block.
