- Replace ADDENDUM 1 TO FAR 52.212-1 in Soliciation FA4819-04-R-4004 posted 24 Jul 2004 on FED Biz Opps, starting on page 36 through page 39 with the following: ADDENDUM 1 TO FAR 52.212-1 Changed 23 Jul 2004 (b) Submission of Offers. Offers must be submitted on SF 1449. (c) PERIOD OF ACCEPTANCE OF OFFER In compliance with the solicitation, the offeror agrees, if this offer is accepted within 120 calendar days from the date specified in the solicitation for receipt of offers to furnish any or all items on which prices are offered at the price set opposite each item, delivered at the designated point)s), within the time specified in the schedule. PROPOSAL SUBMISSION REQUIREMENTS (a) Purpose. These instructions prescribe the format of the proposal and describe the approach for the development of presentation of proposal data. The instructions are designated to ensure the submission of necessary information to provide for the understanding and comprehensive evaluation of the proposal. Offerors shall follow the detailed instructions fully and carefully as received. (b) Evaluation Criteria: This section outlines those areas that comprise the criteria that the government will consider in evaluating the offerors? proposals for the Cable Television Services requirement of Tyndall AFB, FL. An evaluation will be conducted on each proposal received in response to the solicitation. The evaluations will be based exclusively on the content of the proposals and any subsequent clarifications/discussions required. Any information or data incorporated by reference or otherwise referred to will not be considered. Evaluations will be accomplished by comparing offeror capabilities and proposal elements against the government as stated in FAR 52.212-2 (attached), for award in the RFP defines the proposal elements and data required from each offeror for this evaluation. Volumes: The proposal shall consist of original plus one copy with the following items. In addition, include an electronic copy (diskette or CD ROM). VOLUME TITLE NUMER OF COPIES REQUIRED Section I Mission Capability Original plus one and one electronic copy (diskette or CD ROM) Section II Past Performance Original plus one and one electronic copy (diskette or CD ROM ) Section III Price Proposal Original plus one and one electronic copy (diskette or CD ROM) Section IV RFP Documents Original NOTE: Offerors are to return one (1) copy of the attached Representatives & Certifications and all amendments, if issued, with their proposal. Matchkus 501 Illinois Avenue Suite 5 Tyndall AFB FL 32403-5526 (c) Technical Proposal: In addition to the information that you are providing under the basic FAR 52.212-1, a technical proposal is required for evaluation. Technical Proposals shall be limited to 35 pages, which shall be tabbed, divided and indexed to conform to the specific areas, which must be covered in the Proposal (below). Pages in excess of the maximum will not be evaluated. Type shall not be smaller than 12 point and shall be double-spaced, submitted on standard 8.5 x 11 inch paper in a three ring binder with all pages numbered. Tables and Index pages will not count against the total page limitation. The purpose of the technical evaluation is to ensure that the contractor demonstrates a complete understanding of the Statement of Work (SOW) to guarantee that the needs of the government are met. The technical proposal submittals shall address the following: FACTOR A: MISSION CAPABILITY FACTORS: Each subfactor will be evaluated to determine whether the technical offer has provided sufficient information required and to insure that it meets the respective standard. The offeror?s proposal will be evaluated. Each specific criterion is listed with its corresponding assessment criteria. SUBFACTOR 1 ? Channels Offered. Offeror shall submit the listing of channels to be provided and the premium channels for the outlets that require premium channels. SUBFACTOR 2 ? Quality Control Plan. The plan shall include procedures to ensure the requirements of the contract shall be met. SUBFACTOR 3 ? Transition Plan. Submit a plan that provides an overview of procedures and plans for installing the infrastructure necessary to provide cable television services to Tyndall AFB. SUBFACTOR 4 ? Response Plan. Offeror shall provide a detailed plan indicating how it will respond to all service complaints and restore/repair service within the specified time frames. FACTOR B: PAST PERFORMANCE: Information Regarding the Submission of Past Performance Data: Submit written evidence of your experience for the last five years in the areas related to cable television services. Information submitted should contain at least the following for each contract/reference. Limit responses to one page per response. a. Contract number, contract type, (i.e., Fixed Price, Cost Plus Incentive, etc.), indicating whether a government or commercial contract, and dollar amount. Include a summary description of any subcontractor and the scope of involvement. b. The Contracting Agency, including name, telephone number and/or e-mail address of the contract administrator, buyer, or Contracting Officer. c. Type of services provided. d. Summary of quality deficiencies (if any) and action taken for correction. e. To assist the Government in reducing the evaluation period, Offerors shall send the Contractor Performance Assessment Questionnaire (Attachment 3) to ALL past performance references that the Offeror is providing as references in their proposal. When completed by the reference, this past performance questionnaire shall be returned DIRECTLY from the reference to the address listed in block 9 of the SF 1449 on or before the proposal due date and time. Means of Evaluation: The primary means of evaluation will be the Contractor Performance Assessment Questionnaire. However, the government reserves the right to consider any other information obtained through other means, such as Contractor Performance Assessment Report (CPAR) system. FACTOR C: EVALUATION FACTIORS FOR THE PRICE PROPOSAL: The Offeror shall ensure that all prices for every Contract Line Item Number (CLIN) is complete and the Pricing Schedule is completed. Price analysis will consist of the following: 1. The offeror?s price will be evaluated, for award purposes, based upon the total performance price proposed for basic performance period and all options offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise such options. 2. The offeror?s price proposal will be evaluated using one or more of the techniques in FAR 15.404, in order to determine if it is reasonable if it is reasonable and realistic. The Government will evaluate the cost realism of each offeror?s proposed prices. This will include an evaluation of the extent to which proposed prices indicate a clear understanding of solicitation requirements and reflect a sound approach to satisfying those requirements. If the Government evaluates an offer an unrealistically low compared to the anticipated costs of performance, and the offeror fails to explain these underestimated costs, the Government will consider the offeror?s lack of understanding of the technical requirements under the corresponding technical capability subfactor.
