- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation uses Simplified Acquisition Procedures ( SAP) authorized in FAR Subpart 13.5 and is expected to result in the award of one firm-fixed priced contract. The resulting award will be made on SF 1449, for commercial items. This announcement constitutes the only solicitation; quotations are being r equested and a written solicitation will not be issued. W91YTZ-04-T-0140 is a request for quotation (RFQ); its document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-23 and 2001-19 Update. NAICS 453998 (SIC 7389) NON-PERSONAL Services required are for maintenance of therapy pool located at Walter Reed Army Medical Center, Washington DC. Period of Performance will be for base year plus four-consecutive option years, for potential total of five years. PERFORMANCE WORK STATEMENT BASE YEAR PERFORMANCE PERIOD: 16 August 2004 to15 August 2005: DESCRIPTION ITEM 0001: MAINTENANCE SERVICE, ?????? 52 weeks UNIT PRICE:_________Total Amount:_________ The requirement described herein is designed to ensure that the therapy pool (28,500 Gal, Size 20?????? x 42??????) at Walter Reed Army Medical Center is operating at full functioning capacity, maintained and if necessary repaired in accordance with the ma nufacture??????s standard commercial maintenance practices. The contractor shall provide pool supplies, personnel, tools, parts, materials, general and specialized equipment, electronic communication, reporting and transportation necessary for performance of routine maintenance of therapy pool. For any pool equipment or part which may require repair or replacement, e.g., pool pump, contractor must coordinate with Government POC (see para # 1.4.4 for POC information) prior to replacing or repairing part. Co ntractor will vacuum pool, test water for chlorine, PH, alkalinity, calcium hardness and provide necessary adjustments to meet acceptable levels. Of repair parts/materials required to maintain/repair pool equipment exceed $500.00, contractor must notify COR for approval prior to installation/repair of part, and the contracting Officer, in writing, if cost exceeds the ceiling amount of this order. Maintenance shall be performed during regular working hours. Acceptable dates and times to perform services. Thursday 3:00pm ?????? 4:00pm, Friday 9:00am ?????? 11:00am or Friday 3:00pm ?????? 4:00pm, except on American Legal holidays. Maintenance dates will be coordinated with COR upon award.
