21 July 2004 MEMORANDUM FOR RECORD FROM: 375th Contracting Squadron 201 East Winters Road Scott AFB IL 62225-5015 SUBJECT: Site Visit Minutes, FQ442641620100, Install/Remove Carpet B1600 Attendees: Lt Terry Whited, Contracting Officer 256-9289 TSgt Crystal Amos, Contract Specialist 256-9309 Ms. Cicely Hudson, Solutions Mr. Steve Davis, Sonag Inc Mr. Tom Stanely, Sonag Inc I. OLD BUSINESS 1. A site visit was held on 21 Jul 04 at 1000 at building 1600. Two vendors were present. TSgt Amos gave a brief overview of the project. Contractors are to raise the systems furniture and remove and install carpet in the areas specified. As stated in the SOW the work would be done at night between the hours of 1600 and 2400. No work permitted on the weekends. Mr. Hall the end user, will provide temporary passes for the crew. Security Forces personnel will be escorts for the crew. TSgt Amos instructed the individuals that any questions they have would need to be sent in writing by Tuesday 27 Jul 04. She would accept e-mailed or faxed questions. 2. Mr. Hill walked the individuals through the areas and indicated the areas where work will be performed. A vendor tested a piece of carpet and found that it was a quick release. Mr. Hill stated the Government will deliver the carpet in small quantities to for convenience. The contractor has to communicate how much work can be done in 8 hour periods. There will be a storage area in the building for the carpet. 3. A vendor asked about the flexibilty in the communication lines in regard to lifting the systems furniture. They were concerned that the lines are taunt and would be damaged. Mr. Hill stated that the communication lines come from the ceiling and not the floor so there would be no problem but he would double check the location of all the lines. 4. Mr. Hill stated he would have to check and find out if the Commander?s corridor is included in the work. The vendors asked if there were any drawings to scale available for this project? Mr. Hill said that they would check with CE. TSgt Amos would check too. The small drawing provided at the visit would be scanned and uploaded so everyone would have access to it. 5. A vendor stated that the project performance period of 60 days was too short with the amount of furniture that had to be moved during the short hours. He would address this in his questions. 6. A vendor asked if a second site visit could be scheduled so his systems furniture subs could view the site and provide an estimate. He said that he could not determine the amount of furniture to be moved in the package he received. TSgt Amos asked the vendor to put his request in writing and said that she would look into the possiblity of another site visit. This concluded the site visit.
