- Financial Management Service (FMS) is requesting quotations from vendors for the following: CLIN 0001: RDM MICR Verifier MH, Model 6753FK, Quantity = 4 CLIN 0002: 24-month Extended Warranty for the RDM MICR Verifier MH, Quantity = 4. The simplified acquisition procedures set forth in FAR Parts 12 and 13 shall be used. This is a combined synopsis/solicitation. Delivery terms are FOB Destination to (4) FMS locations: Austin, TX; Kansas City, MO; Philadelphia, PA; and San Francisco, CA. The resulting purchase order shall be firm fixed price. The basis for award shall be price alone. All interested parties are requested to submit a written quotation that will be considered by the agency. Quotations may be made on company letterhead. Quotations shall include the company name, point of contact, address, phone number, fax number, tax identification number, DUNS number, business size, delivery terms, proposed unit price, the proposed extended price, and any prompt payment discount terms. If items are available under GSA Schedule, please provide the schedule number. Quotations are due by COB, Wednesday 07/28/2004. Quotations may be faxed to 202.874.0524, Attn: Alicia Steele.
