Description
This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The reference number for this procurement is RFQ 500-47-04 and incorporates the clauses and provisions that are in effect through Federal Acquisition Circular 2001-24. The North American Industry Classification System Code (NAICS) is 339999. Quotes shall be submitted FOB Destination and equipment delivery time should be specified on the quote. The Stratton VA Medical Center, Albany, NY has a requirement for a PTW MP3-S Computer Assisted Linear Accelerator Dosimetry System Utilizing 3D Scanning (Brand Name or Equal). The system will be used for dose distribution measurements and beam analysis in Radiation Therapy and will utilize 3D Scanning of the electron beam. The system will consist of a 142 Liter capacity Water Tank, Full 3D Moving Mechanism which will position to within + 0.1mm utilizing stepping motors for stop and integrate as well as continuous scanning, Two (2) Waterproof Ion Chambers, Control Pendant with visual LED readout of X,Y,Z positioning, Electric Lifting Carriage with Built-in water Reservoir, Leveling Pedestal with 45 & 90 Degree Rotational Accuracy, Control Unit with Tandem Dual Channel Electrometer, Connection & Ionization Cables, Chamber Holders and Clamps, Software for Beam Data Acquisition and Analysis, Treatment Planning Software Module, Operators and Maintenance Manuals, On Site Installation and Training. The system must be compatible with the two existing government owned Linear Accelerators, a Varian Model CL6/100 Linear Accelerator Linear and a Varian Model CL2100 Linear Accelerator. Contractor will provide a one year warranty for materials & workmanship from the date of installation and final acceptance. The scanning system will be used to perform initial commissioning of any new linear accelerators, perform annual mini-commissioning and insure that the linear accelerator is meeting all of the AAPM criteria. It will also perform profile measurements for flatness and symmetry whenever film dosimetry shows flatness and symmetry are out of range. The system will collect data on small electron cutouts to evaluate the effect on the percent depth dose curve and will be used to verify the beam characteristics of an existing MIMIC IMRT delivery system. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-2 Evaluation-Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5 Contract Terms and Conditions Required to Implement Statues or Executive Orders Commercial Items as well as the following addenda, FAR 52-211-6 Brand Name or Equal, VAAR 852.211-77 Brand Name or Equal, 852.211-70 Service Data Manual. Offers including the pricing information are to be submitted on company letterhead and must include a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items, and the company DUNS number. Electronic copies of the referenced provisions and clauses are available on the Internet at www.arnet.gov/far and www.va.gov/oa&mm/vaar/. Technical/Descriptive Literature (Ref. FAR 52.214-21) shall include but not be limited to technical specifications, product pictures, product features, etc., and shall be submitted with the quote or the quote will be rejected as non-responsive. Technical acceptability will be determined by information submitted providing a description in sufficient detail to show that the product meets the government?s requirements. The following factors, in descending order of importance, will be used to evaluate quotes: (1) determination of product acceptability (2) technical ability & experience of offeror in the area of ventilator management systems, including references to customers of existing systems, (3) delivery schedule and (4) price of the item offered to meet the Government's requirement. The Government intends to evaluate the information received and award without discussions. Quotations and all required supporting documentation must be received by 2:00 pm Eastern Standard Time August 10, 2004.
