- NAICS Code: 541710 The Environmental Protection Agency under the Simplified Acquisition Procedures Test Program for Commercial items set forth in FAR Subpart 13.5 intends to issue a sole source purchase order to the following proposed contractor: National Capitol Poison Control Center, c/o Toby Litovitz, M.D., Director. This document represents a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The EPA is FACNET certified. This solicitation is being issued as a Request for Quote (RFQ) under the following request for quotation number: RFQ-DC-04-00249. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-07, effective May 15th, 2004. This solicitation is not set aside for any small business, HUBZone, or 8(a) businesses. Award will be made on an "all-or-none" basis. The NAICS code associated with this solicitation is 541710. Offerors should provide individual firm-fixed-price quotes for the level-of-effort required in the following description. The contract line item(s) will include the following: 1) Electronic data file(s) as described in paragraph marked "description" Description: Electronic data file(s) for the years 2002 and 2003 shall be provided in computer-readable format. These files shall contain information on all human pesticide exposures reported to the Toxic Exposure Surveillance System (TESS) which is maintained by the American Association of Poison Control Centers (AAPCC). AAPCC shall provide the electronic data file(s) for all pesticide exposures. Data shall include cases coded as insecticides, fungicides, rodenticides, herbicides, disinfectants, moth repellents, and fumigants (e.g., methyl bromide, aluminum phosphide, sulfuryl flouride). Quality assurance of all deliverable(s) will be performed prior to acceptance. All inappropriate characters (* ; :) occurring in fields which should only contain numbers shall be replaced with blanks. Files(s) shall be provided in .mdb (Microsoft Access) format. Delivery for this item(s) shall take place no later than six (6) months after contract award. Quote(s) shall be provided FOB Destination.
