- Q-Provide services of eyeglasses and fitters as specified to medical centers listed in VISN 9. VISN 9 consists of medical centers located in Murfreesboro, Nashville, Memphis, Mountain Home (Johnson City) TN; Louisville, Lexington, KY; and Huntington WV. Period of service is one base year with two (one-year) option periods. All responsible sources may submit a proposal which will be considered by the agency. Any requests that cannot be electronically downloaded must be requested in writing to the Contracting Officer. Phone requests will not be honored. This is a total small business set aside in accordance with NAICS code 339115.
