- This is a combined synopsis/solicitation for commercial services (Radiology Services) prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FA9301-04-T-0050. This solicitation is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24 and DFARs Change Notice (DCN) 20040625. The North American Industry Classification System (NAICS) Code is 621111. The size standard is $8.5M. This acquisition is 100%, set aside for small businesses. The acquisition is for the following period 1 August 2004 thru 30 September 2004. Contract Line Item Number (CLIN) Line Item 0001, 3-DAYS A WEEK(8 HOURS PER), ( Monday through Wednesday (Tuesday through Thursday during a federal holiday week)) The Contractor will be required provide Diagnostic Radiologic interpretation for routine plain films, screening & diagnostic mammograms and sonograms to eligible beneficiaries and staff providers for the 95th Medical Group, as defined in the statement of work at Edwards Air Force Base, California.
