Description

AMBULANCE SERVICE, for the James A. Haley Veterans Hospital, Tampa Florida. This is a synopsis for commercial services per the procedures at FAR 12, 13, and 15. Request for Proposals (RFP) Solicitation Number 673-107-04. Solicitation issue date 30 July 2004, proposal due date 23 August 2004, 4 p.m. eastern standard time. The North American Industry Classification System (NAICS) code is 621910, size standard $6M. Contractor to provide all labor, materials, vehicles, equipment and supervision to provide ambulance services for VA beneficiaries. Period of performance: 1 October 2004 (or date of award) through 30 September 2005, with two 1-year option periods. Contractor must be authorized/licensed by Hillsborough County (FL) and the state of Florida, as applicable, to perform trips ordered by the Tampa James A. Haley hospital.
