- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation, RMK0E040085, is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition 2001-03. The North American Industry classification (NAICS) code is 624310 and the business size maximum is $5.0 Million. The proposed contract is set aside for small businesses. All responsible small business sources may submit a quote, which will be considered by the Government. This office is requesting quotes on the following item: 0001: Provide all travel, supplies, materials, and personnel to provide Speech Therapy and Audio-logical Reports for special education students to the BIA, Taos Day School, Taos, NM in accordance with the Scope of Work (SOW). The QUANTITY is One (1), The UNIT is Lump Sum. The Government intends to award a firm fixed price contract. The period of performance is from September 7, 2004 through June 2, 2005 with four option year periods. The SCOPE OF WORK (SOW) is as follows: Scope of Work. SCOPE: The Speech Language Therapist (SLP) designs programs and therapeutic sessions for students that have either a primary or secondary communication disorder in the areas of receptive or expressive language. The therapist also works with children who may have organic impairment such as a central auditory processing deficit, or physical deformations that effect speech and language patterns. Each student who is referred for services receives a battery of tests that are directly related to the area of speech and language in order to decide what the specific needs of the student are. Students are seen as often as 2x a week in a small group or one to one work/therapy session. Weekly notes are kept and are reviewed with classroom and special education teachers, and are an important tool for determination of continued services. The SLP works two days a week for 39 weeks of the school year. The SLP is required to be an active participant of the Educational, Appraisal and Review Team for Special Education. Each student receiving SLP without other special education services needs a completed Individual Educational Plan (IEP), that is kept in a locked filing cabinet. The SLP meets with parents on a monthly basis to discuss progress and to help the parents with any modifications needed for school or the home. There are usually 10 to 15 students served by the SLP during the week in 1 hour sessions. The SLP works with grade level teachers to help modify classroom work that is directly related to audition in the academic areas. The SLP should have experience with Native American children, and possess a Masters level degree in Speech Language Therapy and certified by a professional Speech and Language organization. The contract performance shall begin around mid-August and terminates the first week of June for the base and option year periods. The services of a SLP are mandated by the Individuals with Disabilities Education Act (revised 2002) for students with special needs. The Offeror shall provide the lump sum cost for the base and four option year periods with a breakdown of their cost to provide this service and provide a copy of their certification. The offeror shall have a favorable screening (applicant and suitability) determination must be made by the Bureau of Indian Affairs (BIA) Office of Indian Education Programs (OIEP), Personnel Security and Suitability Program prior to contract award.
