- Contractor to provide all tools, labor and materials necessary for floor mat exchange program including rental and services at the Richard L. Roudebush VA Medical Center, 1481 W. Tenth Street, Indianapolis, IN 46202, in accordance with the solicitation. All floor mats needed for designated locations will be rented by VA with contractor being the owner. Contractor to provide all floor mats, labor and materials required within this floor mat exchange contract. The mats provided shall be of utmost quality, clean, dry and serviced floor mats free of stains to be delivered to designated locations within the facility. Work shall include placement of clean floor mats to designated locations, pick-up existing floor mats for servicing and cleaning. Exchange service shall be performed every seven (7) days (weekly cycle) on Mondays. Contract period will be from 10/1/04 through 9/30/05, with four (4) annual renewal options. This solicitation is 100% set-aside for small business as a commercial item in accordance with FAR Part 12 and FAR Part 13. NAICS code 812332 and size standard $12M applies. All responsible sources may submit a quotation, which shall be considered by the agency. Contractors are responsible for obtaining a copy of all documents (solicitation and amendments) via the website: http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa. Solicitation will be available on or about 8/11/04.
