- The Sierra Pacific Network Consolidated Activity (CCA) is issuing a Commercial Item Request for Proposal in accordance with FAR 12.203 for furnishing two (2) one bedroom suites with fully equipment kitchens to house surgical residents for the VA Sierra Nevada Health Care System, 1000 Locust Street, Reno, NV. Contractor shall provide all utilities, local telephone service, basic cable TV service, furnishings, daily trash removal and daily maid service. Units shall be wheelchair accessible and meet all requirements of the Americans with Disbilities Act. Location of rooms must be within a 20-mile radius of the VA Medical Center. Contract period is October 1, 2004, or date of award if subsequent thereto, through September 30, 2005 with one (1) option year through September 30, 2006. Contract award is subject to availabilty of funds. The following provisions and clauses apply in addition to any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications - Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract Terms and Conditions to Implement Statutes or Executive Orders - Commercial Items.
