- Contractor shall provide Radiation Therapy Services for the VA Medical Center, 4150 Clement Street, San Francisco, CA 94121 for the period of October 1, 2004 through September 30, 2005 with Two (2) one-year options to renew. Services provided shall be defined as professional and technical services with the delivery of external beam treatment including but not limited to all complexities of treatment and planning, associated CT Scans ( or ultrasounds if similarly used) in connection with the course of treatment, Intensity Modulated Radiation Thereapy ("IRMT") when indicated, services of physicists, dosemetrists, radiation therapy technologist, and all Radiation Oncologist Physician Services. Included in these services are Gamma Knife Services. Solicitation packages will be available o/a August 5, 2004 on the following website: https://www.sf.med.va.gov/cca/index.htm No web- based offers will be accepted. No hardcopies of the solicitation will be available. Questions must be in writing and may be emailed to john.gregorio2@med.va.gov or faxed to (925) 372-2044. NO PHONE CALLS WILL BE RETURNED. This is a unrestricted commercial item procurement. The following clauses, provisions and addendum apply: 52.212-1 Instructions to Offerors, 52.212-2 Evaluation, 52.212-3 Representations and Certifications, 52.212-4 Terms and Conditions and 52.212-5 Terms and Conditions to Implement Statutes or Executive Orders.
