FUNDS ARE NOT CURRENTLY AVAILABLE FOR THIS REQUIREMENT. HOWEVER A REQUEST FOR FUNDING IS INCLUDED IN THE FY04 END OF YEAR SPEND PLAN. AWARD IS CONTINGENT UPON FUNDS BECOMING AVAILABLE ON OR BEFORE SEPTEMBER 30TH 2004. ALL QUOTES NEED TO BE VALID THROUGH 30 SEPTEMBER 2004. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number F63CES42030100 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24, Effective 19 Jul 2004. This acquisition is 100% set aside for small business and is under NAICS code 562991, with a small business size standard of $6.0 million. The contractor shall furnish all plant, parts, labor, tools, supplies, equipment, materials and management necessary to provide chemcical toilet service at various areas located on Altus AFB, OK in accordance with the Statement of Need. (580) 481-6602william.belles@altus.af.mil 1 July 2004 STATEMENT OF NEED Chemical Toilets Service 1. DESCRIPTION OF SERVICES: The contractor shall furnish all plant, parts, labor, tools, supplies, equipment, materials and management necessary to provide chemical toilet service at various areas located on Altus AFB, indicated in Appendix A. Provide skilled personnel in compliance with industry standards to perform all work. 1.1. General: All chemical toilets, also referred to as portable latrines, furnished and serviced by the contractor shall be of commercial grade and shall be new to like new at the time of placement and at any time the units require replacing during the duration of the contract. All toilets shall match in color and shall be Federal Standard 595b color #23619 or approved equal. No toilets shall be delivered that have graffiti of any type on the exterior or interior surfaces. The Contracting Officer (CO) or designated representative reserves the right to reject toilets that do not meet the requirements of this SON. Chemical toilets must be in excellent condition at all times. 1.2. Cleaning and Maintenance Service: 1.2.1. Clean chemical toilets to ensure they are scrubbed, sanitized and disinfected at each service occurrence. Cleaning includes exterior surfaces and interior walls, floors, ceilings, doors, urinal troughs and commodes. The contractor shall also clean the immediate area, within a five (5) foot radius, of all trash/debris at each service cycle. Toilets shall be supplied with an adequate amount of toilet tissue to ensure supplies do not run out between servicing. Unless indicated otherwise, chemical toilets shall be serviced no less than two times per week, typically Tuesday and Friday. At each service frequency waste material shall be pumped out and replaced with a fresh chemical solution. All waste material shall be disposed of off Altus AFB at an approved disposal site in accordance with local, state and federal guidelines. The contractor is responsible for all permits and fees. 1.2.2. Minor repairs to toilets shall be performed at each service cycle. Major repairs shall be accomplished off site. Toilets requiring major repairs shall be replaced with new. The contractor shall replace any toilet that is declared unserviceable by the CO or designated representative. Replacement shall occur within twenty-four (24) hours. 1.2.3. The contractor shall, at his own expense, repair all damages to government property resulting from placement, movement or replacement of toilets. Repairs shall be made by craftsmen of the particular trade in an approved manner. 2. SERVICE DELIVERY SUMMARY: 2.1 General: The contract service requirements are summarized into performance objectives that relate directly to essential services. The stated performances thresholds are the minimum acceptable levels of service for each stated requirement. All aspects of the contract performance are subject to government surveillance and absence of any contract requirement from the SDS shall not detract from its enforceability nor limit the rights or remedies of the government under contract clauses. 2.2. GOVERNMENT FURNISHED PROPERTY AND SERVICES: The government shall only furnish areas of Altus AFB for placement of chemical toilets. 4. GENERAL SERVICES: The contractor shall provide the name of their representative with the authority to act for the contractor for all operations and emergencies. Contractor shall supply the Contracting Officer (CO) and Quality Assurance Evaluator (QAE) with a phone number, home and mobile and will call the Government Evaluator within 3 hours of notification of any problems during normal duty hours, 0730 through 1630 hours. It is not necessary to physically show up to be considered responsive. Contractor shall service the toilets at a minimum of twice each week and perform routine maintenance and cleaning in accordance with all base, state and federal regulations. The contractor shall physically check with the QAE prior to 0830 hours on the day that maintenance is scheduled to collect service calls. Upon completion of service the contractor shall provide a completed list of repairs that were made to each toilet to the QAE. The contractor shall repair or replace damaged or unserviceable toilets. The contractor shall repair, at his expense, any damage to government property from the servicing, placement or movement of toilets. 4.1. Placement: All portable latrines furnished under this contract shall be placed in position during the first two (2) calendar days of the contract period. At the pre-performance conference the contractor shall submit a schedule for placement of the toilets. Such schedule shall indicate the dates latrines will be installed by locations. 4.2. Phase-In & Phase-Out: Phase-in of equipment shall commence from 0700 hrs through 1600 hrs. The installation/placement shall be completed not later that two (2) calendar days from contract start date. The schedule may be altered to accommodate cooperation of contractors phasing in and phasing out provided the contractor ensures that no area is without a toilet for more that 24 hours. The contractor shall supply all necessary repair parts, tools, equipment and vehicles to perform proper placement/service for the life of this contract. Phase-out shall be completed within 2 calendar days following contract completion and from 0700 hrs through 1600 hrs. 4.3. Vandalism: The contractor shall notify the QAE when vandalism to toilets is suspected. The QAE and contractor shall inspect the toilet(s); and a report, completed by the contractor, shall be sent to the CO. All cases of vandalism will be made on a case-by-case basis by the CO who shall be the final deciding authority and compensation may be allowed at that point. 4.4. Compliance With Environmental Laws: The contractor and subcontractor shall comply with all applicable federal, state and local laws, regulations, ordinances, policies and standards related to environmental matters. Contractors and subcontractors shall also comply with all applicable specific instructions, policies or references contained herein. Policies and procedures can be reviewed at the Base Environmental Coordinators office on Altus Air Force Base. 4.5. Hazardous Materials: Hazardous Materials (HM) are defined in 49 CFR 172.101 and 40 CFR 301-304. Absolutely no HM shall be brought onto Altus AFB until that material is authorized for use by the Hazmat Pharmacy in building 228. The contractor shall submit AF Form 3952, Material Safety Data Sheets (MSDS) and other supporting documentation for each HM used prior to their use. If encountered, the contractor shall notify the Contracting Officer and HAZMAT Pharmacy of any material not identified in the contract documents thought to be hazardous. The Government shall be responsible for characterization, storage transportation and disposal of the unidentified waste, if necessary. 5. Each toilet shall be equipped with fold down or fixed steps to facilitate safe access and use. Trailers shall have a ?lunette? eye on the towing arm that can be inserted into a ?pintle? hook on vehicles. All maintenance and repair to trailers, to include repairs of flats or replacement is the responsibility of the contractor.
