Description
This is a combined synopsis/solicitation for Commercial items prepared in accordance with the format in FAR subpart 12.6 and supplemented with additional information included in this notice. This announcement constitutes the solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is F5MED41830100. The solicitation is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24 and DFARS Change Notice 20040625. All responsible sources shall submit a written quote, which will be considered. Facsimile submissions will also be accepted. This combined synopsis/solicitation is UNRESTRICTED. Item/Services Description: 1. The contractor will provide the following work: a. Convert x-ray system GE ADVANTX, model number: 2121197 from 60-hertz operation to 50-hertz operation. b. Perform post conversion preventive maintenance inspection. c. Perform post conversion operational inspection. d. Perform post conversion calibration / verification. 2. The contractor will provide the following: a. All specialty tools to perform the above work. Basic tools will be available locally. b. All system software required to perform the above work. c. All hardware required to perform the above work. d. All manuals required to perform the above work. e. All test equipment required to perform the above work. 3. Upon contract award, the contractor must: a. Send all required personal information, including full name, date and place of birth, passport number and place of residence to obtain installation gate pass to the contracting office. All information will be required at least two weeks prior to visit or have a military ID card, which will grant him/her access to the installation. b. Be available at Lajes Field for the proposed period of performance during the week of 13 ?20 September 2004. The Lajes Field base power conversion will take place on 18 Sep 2004. Contractor must propose amount of time necessary to perform pre-conversion work and perform post conversion preventive maintenance inspection, operational inspection, and calibration / verification. Note: If the X-Ray system does not operate correctly at 50 Hz, because the necessary parts/software were not converted/updated to meet the 50 Hz electrical requirement, the contractor is responsible to update any parts/software to make it operate correctly at their own expense. PROPOSAL PREPARATION INSTRUCTIONS: The proposal shall have 2 line items. Line item 0001 shall include all costs for the labor and parts to be converted/updated, Line item 0002 shall include all costs for Travel (eg. Airfare, Rental Vehicle, Hotel) and any other Miscellaneous Expenses. The provision at 52.212-1, Instructions to Offerors - Commercial items, applies to this acquisition. Offerors shall provide a firm fixed-price offer. FAR 13.106-2, Evaluation of quotations or offerors The Government will award a contract resulting from this solicitation to the responsible offer whose offer conforming to the solicitation will provide the best value to the government. The government reserves the right to make award to other than the low bider. Successful contractor must meet all the responsibility requirements of Subpart 9.1 of the Federal Acquisition Regulation (FAR). A written notice of award or acceptance of offer, mailed or otherwise furnished to the successful offeror, shall result in a binding contract without further action by either party.
