- Description
- The Centers for Disease Control and Prevention (CDC), Office of the Director, Office of Health and Safety (OHS), headquartered in Atlanta, Georgia, contemplates awarding a contract to obtain the services of a Contractor to provide a comprehensive program of occupational health services. These services will include (1) In-service physical examinations, (2) Preventative and health promotion programs, (3) Occupational health surveillance programs, (4) Employee counseling services, and (5) Work-site health promotion activities for employees in the Atlanta area; to include the Agency for Toxic Substances and Disease Registry (ATSDR), a sister agency that shares the CDC infrastructure. At CDC’s discretion, the Contractor may be a central focus for agency-wide occupational health services which may extend beyond the Atlanta area. Award of a Cost-Plus-Fixed-Fee contract is contemplated, in accordance with FAR Part 15, with Full and Open Competition. The anticipated Period of Performance is as follows: Basic Year - April 1, 2004 to March 31, 2005 with four option years. CDC will consider responsible sources responding to this Request for Proposal (RFP). All questions concerning this RFP shall be submitted via e-mail to ngw5@cdc.gov; Nancy Ware, 770-488-2878. The tentative issue date is January 15, 2004, the RFP closing date is March 1, 2004. The complete solicitation package will be available and posted at this site for downloading when issued.
