- Description
- (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation of Purchase Request F73HOS41960400 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24. (iv) This acquisition is for full and open competition. NIACS Code 562111. Size Standard $10,500,000.00 (v) Line item 0001 BASIC YEAR 1 Oct 2004 ? 30 Sep 2005. QTY 12 Months. Line item 1001, Option Year Two, 1 Oct 2005 ? 30 Sep 2006. QTY 12 Months Line item 2001, Option Year One, 1 Oct 2006 ? 30 Sep 2007. QTY 12 Months Line item 3001, Option Year Three 1 Oct 2007 ? 30 Sep 2008. QTY 12 Months. Line item 4001, Option Year Four, 1 Oct 2008 ? 30 Sep 2009. QTY 12 Months (vi) This solicitation is for refuse disposal of one (1) Sani-Pak roll over dumpster, contains approximately 40 cubic yards of waste. Frequency of Service will be once per week or as needed. Contractor?s submitting proposals shall have proper equipment to hook to a Sani-Pak machine and have access to a landfill. The contractor shall secure and pay for all required permits, fees, and licenses required to dispose of waste from the Sani-Pak Dumpster. (vii) Performance Period shall be 1 Oct 2004 through 1 September 2005 (plus four option years). Service will take place at the 3rd Medical Group, 5595 Zeamer Ave, Elmendorf AFB, AK 99504. (viii) FAR clause 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. (ix) FAR clause 52.212-2, Evaluation--Commercial Items, is not applicable. (x) Offerors are advised to include a completed copy of the provision at FAR clause 52.212-3, Offeror Representations and Certifications-Commercial Items, with the quote.
