Description

This is a ?NonPersonal Services? for the contractor to provide the services of five full-time equivalent (FTE), fully licensed Physicians/Primary Care Physicians. The definition of FTE is a contractor employee working 8 hours per day, 5 days per week. The current ?minimum? requirement is five (5) full-time equivalent physicians and the estimated ?maximum? requirement is ten (10). The requirement for full-time equivalent physicians maybe provided with a combination of two or more physicians. Primary care services are required at the following sites; Chalmers P. Wylie Outpatient Clinic (VAOPC), 543 Taylor Avenue, Columbus, OH 43203-1275, Zanesville Community-Based Outpatient Clinic (CBOC), 840 Bethesda Drive, Building 3A, Zanesville, OH 43701, and Grove City Community-Based Outpatient Clinic (CBOC), 1955 and 1957 Ohio Drive, Grove City, OH 43123. These outpatient facilities are managed and directed by the Director or designee of the Department of Veterans Affairs Chalmers P. Wylie Outpatient Clinic, 543 Taylor Avenue, Columbus, OH 43203-1278 hereinafter referred to as VA or VAOPC Columbus. The VAOPC currently operates an Alternate Delivery Site (ADS) at 1914 Tamarack Road, Newark, OH 43055 and may potentially manage a Field Service Center (FSC) at Corporate Center II, 1203 Delaware Avenue, Marion, OH 43302 there may be a need for contract physician services at one or both sites in the near future. The VAOPC reserves the right to add these 2 sites and any additional outpatient facility sites that may be established and fall under the management of VAOPC during the term of any resulting contract.
