- Description
- The Centers for Disease Control and Prevention intends to procure assistance in the review and evaluation of current programmatic cervical cancer screening policies and facilitation in updating specific policies. A complete description of the requirement is available in a Statement of Work. The Government intends to evaluate offers and award a purchase order without discussions with offerors providing the requested service. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. The North American Industry Classification (NAICS) Code is 541611 and the Size Standard is $6,000,000 for this requirement. This requirement is a SMALL BUSINESS SET-ASIDE. Interested small business concerns capable of furnishing the Government with the service presented in this synopsis should submit two (2) copies of a proposal to the address below in response to this Request For Proposal (RFP). The proposal shall include itemized pricing, prompt payment terms, the F.O.B. Point (Destination or Origin as applicable), Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNs), and description of the service proposed. The proposal shall include reference to RFP No. 11043. Proposals will be due August 23, 2004, 12:00 PM (EST). All responsible sources may request a Statement of Work and submit a proposal which, if timely received, shall be considered by the Agency. Email is the preferred method of request for the Statement of Work (SOW). Send requests for the SOW to eem4@cdc.gov with RFP No. 11043 referenced in the subject line. Proposals must be submitted in writing to the Centers for Disease Control and Prevention, 2920 Brandywine Road, Atlanta, GA 30341, Attention: Lenore Mickle. Email proposals will be accepted if dated and signed by an authorized company representative to eem4@cdc.gov. NOTE: The contractor must have an active registration with the Central Contractor Registration (www.ccr.gov) at the closing of this RFP in accordance with FAR 52.204-7. This action is pursuant to 41 USC 253(1) and under the authority of FAR 6.302-1. This requirement will be procured under the Federal Acquisition Regulation (FAR) for Simplified Acquisitions, FAR 52.213-4 Terms and Conditions - Simplified Acquisitions (Other Than Commercial Items). Proposals are due August 23, 2004 by 12:00 PM (EST) at the Centers for Disease Control and Prevention; Construction and Facilities Management Branch; Attn: Lenore Mickle; 2920 Brandywine Road, MS K-71, Atlanta, GA 30341.
