- This is a combined synopsis/solicitation for commercial items prepared in accordance with format Subpart 12.6, as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requisition number, FA4427-04-Q-0018, is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24 and Defense DCN 20040625. The acquisition falls under North American Industry Classification System (NAICS) code 561110 and the associated business size standard is $5Million. If two (2) or more responsive, responsible and technically acceptable small or HUBZone business respond, this acquisition will be100% set aside for small business, if not, it will be unrestricted. The government intends to solicit and award this purchase order for the following item: ITEM 0001, CONTRACTOR TO FURNISH ALL LABOR, MATERIALS, TOOLS, TRANSPORTATION AND ALL NECESSARY ITEM TO PROVIDE SERVICE AS AIRMAN ATTIC COORDINATOR for the Family Support Center. Contractor shall provide services twenty four (24) hours per week for a maximum of 1,236 hours annually in the management and operation of the Airmen?s attic. Services included but not limited to receiving donated items, sorting donations, stocking shelves, assisting customers, recording computerized customer statistics and supervising volunteers. Service to be provided 1 October 2004 through 30 Sep 2005. This solicitation contains one line item: FOB point is Travis Air Force Base, Ca 94535, Bldg 660. The following provisions or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructors to Offerors-Commercial. FAR 52.212-2 Evaluation ?Items, paragraph (a) to this provision is completed as follows ?Evaluation Factors: Price only. FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for award. FAR 52.212-4, Contract Terms and Conditions- Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statutes or Executive Orders?Commercial Items include clauses (b) (14),(16),(17),(18),(19),(30) (c)1, (2) Statement of Equivalent Rates for Hires-filled in as follows: Wage Determination 94-2069 Revision 23, Date of Last Revision 06/17/2004, General Clerk, $12.10 Per Hour. If HUBZone or small business apply clauses at 52.219-3 or 52.219-6 will be included into resultant award. Vendors must be actively registered with the Central Contractor Registration (CCR). The CCR website is http://www.ccr.gov. Contractors are asked to submit their TAX ID number with their proposal. Quotes must be valid for 60 days. Quotes may be transmitted by e-mail or FAX to 707 424-2712 and are to be received at the 60th Contracting Squadron no later than 4:30PM PST on 27 Aug 04. STATEMENT OF WORK AIRMAN?S ATTIC COORDINATOR 1. SCOPE The Travis Family Support Center requires a contractor to provide for the support and operation of the Travis Air Force Base Airmen?s Attic. The services, primarily directed at married senior airman and below and single parents who are senior airman and below. The contractor will coordinate with Federal, State, and civilian agencies for disposition of property as required, and market the program services to the target population. 2. PERSONAL RESUME. Interested individuals shall submit to the Government their official resume. The resume shall not be more than two pages in length, and must contain the following information: 2.1. Full name, address, and home telephone number 2.2. Chronological work experience that substantiates by involvement and duration the skills and abilities that are required in this position. 2.3. A dated and signed statement by the individual certifying that the information in the resume is true and accurate. 3. PERFORMANCE. Contractor will, between 1 October 2004 and 30 September 2005: 3.1. Provide 24 hours of service per week for a maximum total of 1,236 hours annually. 3.2. Implement and follow through on the operation of the Airmen?s Attic in accordance with the Airmen?s Attic Operations Guide, including but not limited to: 3.2.1. Opening the Airmen?s Attic to eligible population each Wednesday and Friday from 10:00 a.m. until 2:00 p.m. to include updating the computerized record of items issued to customers. The Airmen?s Attic Coordinator should arrive one hour prior to opening and should remain one hour after closing. 3.2.2. Working on site, on Tuesday and Thursday when Airmen?s Attic is not open to customers, includes sorting, receiving, and the arrangement of donated resources. This work must be done during normal operating hours of the Family Support Center. 3.2.3. Recruit and supervise other volunteers while working at the Airmen?s Attic. 3.2.4. Immediately report to the Superintendent any unsafe conditions in the Airmen?s Attic or any knowledge of the sale of any Airmen?s Attic items. 3.2.5. Pick up donated items from Base Exchange when they are offered. 3.3. Provide a report to the Quality Assurance Evaluator by the fourth working day of each month indicating: number, rank, and marital status of clients, names of volunteers, volunteer hours of service, and total hours of operation of prior month. Check with the Superintendent for Airmen?s Attic items donated during attic closure. 3.4. Invoice monthly for services rendered. 4. GOVERNMENT SUPPORT. The Family Support Center will provide printing support for material as required: be available to assist the contractor, provide working space, FSC calendar, pencils and pens, filing and storage space, copy of Airmen?s Attic Operating Procedures and computer. 4.1. The points of contact for contracts with the Family Support Center will be the Director, Joan Bueto or Superintendent, MSgt Lucinda Matteson, 351 Travis Ave, Travis AFB, CA 94535-2045, 424-2486. Once the contract is awarded, the point of contact will be Mr. Richard Durrenberg. . 5. PERSONNEL QUALIFICATIONS. 5.1. The Airmen?s Attic Contractor must be familiar with and be responsive to military protocol, organization and structure, jargon, and chain of command. Either prior military experience or having provided this type of service under a prior military contract shall evidence this knowledge. 5.2. The Airmen?s Attic Coordinator must have a working knowledge of computer programs used for Airman?s Attic operations.
