- (i) This is a synopsis for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requeste d and a written solicitation will be issued. PAPER COPIES OF THIS SOLICITATION WILL BE AVAILABLE UPON REQUEST AROUND 23 AUGUST 2004. (ii) Solicitation W9124M-04-T-0099 will be issued as a Request for Quotation (RFQ) using Simplified Acquisition Procedure s. (iii) Incorporated provisions and clauses are located in the Federal Acquisition Regulation 12.6. It is the contractor's responsibility to be familiar with all applicable clauses and provisions included in the solicitation. (iv) This procurement is 100 % Set-Aside for Small Business. The SIC is 8322; NAICS is 624190, setting the size standard to $6.0M. (v) Contract Line Item Number (CLIN) 0001: Partners in Prevention (PIP) Specialist. Resultant contract will be awarded for one base year, with option to extend contract two, one-year option periods.
