Description

Establishment of a Community Based Outpatient Clinic (CBOC). The contractor shall provide Primary and Preventive Medical Care, Mental Health Care and the continuity of that care for identified VA beneficiaries of the William Jennings Bryan Dorn , Department of Veteran Affairs Medical Center in Rock Hill, SC that fulfill the VA's healthcare delivery requirements, in accordance with all terms, conditions, and provisions of the solicitation. The offeror/basic provider will be expected to sub-contract for any required service, that it does not provide, at no additional cost to the Government. It is the VA's intention to solicit proposals on a competitive basis and award a contract that provides for a capita rate method of payment for Contract items. Costs shall represent an “All Inclusive” reimbursement capita rate per patient, per month. Mental Health services will be provided by the Contractor and shall be priced separately as an “All Inclusive” rate per visit, per patient. Other services will include care for non-enrolled patients who may reside in the CBOC catchment area and referred to the CBOC by the Dorn VAMC or the Greenville Outpatient Clinic on patients treated by these facilities who may live in the area where the contractor is providing contracted service. Veterans receiving services under this contract reside primarily in the catchment area of Rock Hill, South Carolina. The counties include Cherokee, Chester, Lancaster, Union, and York. Contractor will provide services at a facility that must have adequate spacing for work required in the solicitation. The contract includes a Base Year (one year from date of award) plus 4 one-year renewal options that may be exercised at the option of the Government. The total duration of the contract(s) will not exceed 5 years. This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. RFP 247-0312-04 is advertised as full and open competition with NAICS Code 621498 with the size standard of $8.5 million. The solicitation will be issued o/a 5/14/2004. A preproposal conference will be scheduled o/a June 4, 2004. The exact date, time, and location will be submitted to all interested parties upon written request to the Contracting Officer.
