- Naval Hospital Pensacola FL has a requirement for an on-site maintenance agreement for government owned CO2 Laser Machines for the period 04 OCT 01 - 05 SEP 30 with three additional option years. Requests for the solicitation shall be accepted by e-mail or at the NECO Website. Bids/quotes shall be accepted by fax, e-mail, or hard copy. Point of Contact for this requirement is Wanda J. Maintenance - Contractor to provide Preventive Maintenance on Surgical Dermatologic CO2 Laser. Shapland Model #2025, S/N 015-03335. 12 Month 0002. Maintenance - Contractor to provide Preventive Maintenance on Surgical Detmatologic CO2 Laser, Laser Engineer Model #MD50, S/N 1202. 12 Month 0003. Maintenance - Contractor to provide Preventive Maintenance on Surgical Dermatologic CO2 Laser. Sharplan Model #1025, S/N 27-117A. 12 Month Services to be performed at the Naval Hospital Pensacola, 6000 West Highway 98, Pensacola Florida 32512. STATEMENT OF WORK GENERAL: The Contractor shall provide Preventive and Corrective Maintenance in accordance with original manufacturer?s recommendation and schedule during normal working hours 8:00AM to 5:00PM, Weekdays, except Government Holidays for the period of October 1, 2004 to September 30, 2005. All Maintenance and Service shall be performed by Competent, Professionally Trained and Technically Qualified Service Technician. The Contractor shall provide a Field Service Report for all work actions to Biomedical Engineering Division upon completion of work. Such documentation shall be in accordance with current regulations and industry standard practice. This Preventive Maintenance Contract covers two (2) inspections and one floting service call per equipment. The PM inspections and Floating Service Call include labor and travel. Parts needed during PM and service calls are not included in this contract and will be billed at current list price on a separate order. The contractor will notify Biomedical Repair Department and request authorization before replacing any repair parts. Preventive Maintenance checks will include calibration and alignment of Laser Unit to manufacturer?s specifications, and check all fluid levels. PERFORMANCE: The Contractor shall be called and respond to Floating Service Call requests by telephone withing twenty-four (24) hours, on-site within forty-eight (48) hours. WORK VERIFICATION AND MAINTENANCE RECORDS: The Contractor shall ONLY respond to Service Calls placed by Biomedical Engineering Personnel. For equipment located in the Naval Hospital, Building 2268, The Contractor shall check in at the Supply Building for a temporary Vendor Badge, and then to Biomedical Engineering Division prior to performing any work. For equipment not located in the Naval Hospital, Building 2268, The Contractor shall be given a point of contact when the service call is placed. Upon completion of the work the Contractor shall give the point of contact a signed legible copy of the service report. The point of contact will then submit the service report to the Biomedical Repair Division. Point of Contact: The Contractor shall provide a point of contact for all maintenance that is required. This information shall include the name, address, and telephone number of contractor service personnel. Credentialing: The Contractor shall provide documentation indicating that their Service Technician is Certified/Qualified to perform such maintenance and or repairs. NOTE: All Contractors must be registered in the Central Contractor Registration database (CCR), to be eligible for Department of Defense contract awards. All responsible contractors may obtain a copy of the solicitation and submit a bid.
