Description
This is a synopsis for commercial items prepared in accordance with the format under Simplified Acquisition Procedures in accordance with FAR Part 13.5, as supplemented with additional information included in this notice. The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-27. This acquisition is NOT small business set-aside and the North American Industry Classification System (NAICS) code is 519120. It is the intent of the National Library of Medicine (NLM) to procure professional services to duplicate microfilm for the NLM collection and to fill requests from individuals and institutions to purchase microfilm. Most of the reels to be duplicated for NLM are camera masters produced from the 1940s to 1985 that do not conform to current preservations standards. Duplication to fill third party requests may be from camera masters or print masters. All duplication work must meet national standards and guidelines for preservation microfilm. The tasks to be performed include the following: 1) evaluate the physical condition, density and resolution of NLM camera master and print master microfilms; 2) check information on box labels against actual content of the microfilm; 3) produce second- and/or third-generation microfilm duplicates for NLM's collection; 4) provide microfilm duplication, prompt shipping, and direct billing for authorized third party purchasers and independently resolve any questions or problems related to third party invoices, payments or shipments; 5) spool, secure with string-tie wrapper, and box and label the duplicated film; 6) inspect duplicated films to ensure optimal copying and viewing quality; 7) check contents, packaging and labeling prior to shipping duplicates to NLM or third party sales requestors; 8) submit monthly reports and invoices; and 9) provide storage and work areas with suitable temperature and relative humidity and adequate protection against fire, flood, theft and other risks. The purchase order term will be September 30, 2004 through August 29, 2005 with options for four (4) successive one-year periods. The FAR provisions and clauses sited herein are incorporated by reference into this solicitation. Offerors are advised to propose in accordance with the provision of FAR 52.212 1, Instructions to Offerors Commercial Items. Offerors are advised to provide with their offer a completed copy of the following provisions: FAR 52.212 3, Offeror Representations and Certifications Commercial Items (http://www.arnet.gov/far/). The following FAR clauses apply to this acquisition: FAR 52.212 4, Contract Terms and Conditions Commercial Items; FAR 52 212 5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items. Request for Quotations (RFQ) Number NLM-04 198-UHP will be available on or about August 26, 2004. Request for RFQ-NLM-04 198-UHP may be obtained by writing to the National Library of Medicine, Office of Acquisitions Management, 8600 Rockville Pike, Building 38A, Room B1N20, Bethesda, Maryland 20894; Attention: Uyen H. Phuong, Lead Purchasing Agent. Facsimiles WILL NOT BE ACCEPTABLE. All responsive sources may submit a proposal which will be considered by the NLM.
