Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice and following the simplified acquisition procedures in FAR part 13.5. This announcement constitutes the only solicitation: proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number 6TSA-04-JMT-0098 is being issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-14. This procurement is for the following: CLIN 1: KIP Model 6000 printer, QTY.1; CLIN 2: KIP model 1020 printer, QTY.5; CLIN 3: Four years of on site maintenance.2000 sheets per month per machine. CLIN 4: Quickscan Pro Level 3 HIG Crom Image scan/printers/store for Windows, #QS PRO/3, QTY. 2. DROP SHIPPED IN JAPAN. Two of the model 1020s will be deliver to Sasebo, Japan and the remaining items will be deliver to Yokosuka, Japan. This requirement is for the Department of the Navy, US Naval Ship Repair Facility, Japan. Please include all requirements on quotations. No partial quotes. No substitutions. The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. Evaluation is based on best value including past performance, cost and ability to meet service requirements. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at FAR 52.212-4, Contract terms and Conditions-Commercial Items, applies to this acquisition. In addition to the guidance contained in FAR 52.212-4, contractors shall submit proof of delivery with their invoices. Proof of delivery may be in the form of a bill of lading or other shipping document signed by the receiving office. Invoices received without proof of delivery will not be accepted. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. Offers are due by close of business August 23, 2004 and can be submitted by fax to 816-823-3277, Attn: June Thomas or email june.thomas@gsa.gov.
