Description: Provide polysomnography shall be provided to veteran beneficiaries for diagnosis and follow-up of Obstructive Sleep Apnea, Insomnias, Narcolepsy and all other recognized sleep disorders. Offer's facility location will be within the 30 miles of the VA Medical Center located at 800 Hospital Drive, Columbia, MO 65201. Successful offeror needs to schedule appointment for veteran patients no later than 14 calendar days after notification by the VA personnel. NAIC code is 813920.
