- This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6 as supplemented with the additional information included in this notice. This Solicitation number is W9124J-04-T-0066 and i s issued as request for quotation (RFQ). The North American Industry Classification System (NAICS) Code is 485310. DESCRIPTION: Southern Region Contracting Center West, Fort Sam Houston, ATTN: SFCA-SR-CC, Building 4197 2107 17th Street, Suite 15 Fo rt Sam Houston, Texas 78234-5015, intends to procure Cab service for Applicants Processing at the Houston Military Entrance Personnel Station. Transportation To and From 701 San Jacinto Houston Texas to Crown Plaza 6701 S. Main and the following Medical C onsult Facilities :1315 Calhoun, 952 Echo Lane, 6411 Fannin ,6624 Fannin, 7505 Fannin, 909 Frostwood, 5620 Greenbriar, 1213 Herman, 820 Holman, 8830 Longpoint, 8850 Longpoint, 1200 Mckinney, 1315 St Joseph Parkway, 1401 St Joseph Parkway, 7734 SW Freeway, 7777 SW Freeway, 910 Travis, 3100 Weslayan, 9034 Westheimer, 5420 Dashwood, 1300 Moursound, 1806 W. Alabama, 8200 Wednesbury. The Contractor shall provide Cab Service for the Houston Meps 701 Jacinto Houston Tx. Contract period shall be from October 1, 2 004 through September 30, 2005 with an additional two Option Years thru 30 Sep 2008. Award will be made based on utilizing Simplified Acquisition Procedures for Commercial Items. The Provision at FAR 52.212-1, Instructions to Offerors-Commercial, without addenda, applies to this acquisition. Offerors shall include a complete copy of the provision at FAR 52.232-3, Offeror Representations and Certification Commercial Items with offer. The provisions at FAR 52.212-4, Contract Terms and Conditions Commercia l Items, with addenda, FAR 52.212-5, Contract Terms and Conditions with addenda, apply to this acquisition and FAR 52.216-2, as amended by Contracting (including 52.222-35; 52-222-36; 52-222-37; 52.222-41; 52.222-42; 52.222-47; 52.217-8 and 52.217-9).
