Description
The Bureau of Prisons is seeking a Request for Quote from sources having the ability to provide OUT-PATIENT assessment and counseling services(Line Item #1) and Mental Health Services (Line Item #2) within two-three miles of the Pharos House CCC and on a local public transportation route, located at 5 Grant Street, Portland, Maine for drug abuse treatment services and mental health services for male and female Federal offenders (inmates) held under the authority of various United States Statutes. Performance period under this contract will be for a one year base period to begin on October 1, 2004 with four (4) one year option periods, with service ending (if all option years are renewed) on September 30, 2009. The services required for this contract include Line Item #1: Base Period: October 1, 2004 to September 30, 2005. Estimated Number of Inmates 18. (18) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 26 Individual units x 18 inmates = 468 total units; Group Counseling Sessions (30 minute sessions per inmate) 47 Group Units x 18 inmates = 846 total units. Option Year One: October 1, 2005 to September 30, 2006. Estimated Number of Inmates 19. (19) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 26 Individual units x 19 inmates = 494 total units; Group Counseling Sessions (30 minute sessions per inmate) 47 Group Units x 19 inmates = 893 total units. Option Year Two: October 1, 2006 to September 30, 2007. Estimated Number of Inmates 20. (20) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 26 Individual units x 20 inmates = 520 total units; Group Counseling Sessions (30 minute sessions per inmate) 47 Group Units x 20 inmates = 940 total units. Option Year Three: October 1, 2007 to September 30, 2008. Estimated Number of Inmates 21. (21) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 26 Individual units x 21 inmates = 546 total units; Group Counseling Sessions (30 minute sessions per inmate) 47 Group Units x 21 inmates = 987 total units and Option Year Four: October 1, 2008 to September 30, 2009. Estimated Number of Inmates 22. (22) Intake Assessments (one per inmate); Individual Counseling Sessions (30 minute sessions per inmate) 26 Individual units x 22 inmates = 572 total units; Group Counseling Sessions (30 minute sessions per inmate) 47 Group Units x 22 inmates = 1,034 total units Line Item #1. For Line Item #2: Base Period, Option Year One, Option Year Two, Option Year Three, and Option Year Four, respectively: Estimated Number of Inmates 5. (2) Mental Health Intake Assessment and Report (1 per inmate). (10) Medication Monitoring (two per inmate)for Line Item #2. The services described for Line Items 1 and Line Item 2, shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work, dated January 2004. The estimated closing date is o/a June 10, 2004. All responsible sources may submit a quote which will be considered by this Agency. The RFQ will be posted o/a May 10, 2004. Responsible sources shall submit a proposal for Line Item 1 (Transitional Drug Abuse Treatment Services), or Line Item 2 (Mental Health Services), or both Line Items.
