- Description
- The Operational Contracting Division, Commercial Acquisition Branch at Hanscom AFB, is issuing a Request for Proposals (RFP), FA2835-04-T-0021 for a Women's Health Nurse. The duration of this Simplified Acquisition will be done for the Period of Performance of one (1) Basic Year from 01 Oct 04 through 30 Sep 05 and Four (4) Option Year Periods, Option Period One, 01 Oct 05 through 30 Sep 06; Option Period Two from 01 Oct 06 through 30 Sep 07; Option Period Three from 01 Oct - 07 through 30 Sep 08 and Option Period Four from 01 Oct 08 through 30 Sep 09. This procurement is a Small Business Set-Aside. The contractor shall furnish Women's Health Nurse Services for Government beneficiaries at the 66th Medical Group Health and Wellness Center at Hanscom AFB, MA 01731. These duties will include all labor, education and training, recommendations, consultat ions and reports. The provider shall work within Clinic duty hours, not to exceed 2080 hours per Fiscal Year. Health Insurance is not provided. Performance shall be in accordance with the requirements provided in the Performance Work Statement. All respondents must complete FAR 52.212-3, Offeror-Representations and Certifications-Commercial Items to be submitted with proposals. This may be accessed at: http://farsite.hill.af.mil. Award will be made to the offeror whose offer is the best value to the Government considering price and past performance of equal importance. Offerors are required to submit at least three (3) references for evaluation of past performance. The Offeror's Package should include a complete resume and all documentation of required licensing and certification. The Basic Year and all Four (4) Option Years should be responded to separately with inflation taken into consideration. The contractor will be paid only for hours worked. All line items must be filled in to receive an award. Proposals must be received no later than 01 Sep 04 at 3:00 PM (local time). Be advised that all interested parties must be registered in the Central Contractor Registration (CCR) Database in order to receive an award.
