Description
RFQ 11501-0027-4 is issued as a Request for Quotation for commercial items in accordance with the format in Federal Acquisition (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2001-24. This action is unrestricted. The North American Industrial Classification System Code is 424490 with a size standard of 500 employees. The Government intends to make a single award of a fixed price, indefinite delivery/ requirements type contract with firm-fixed unit pricing for the provision of various fountain drink products and equipment for the Federal Prison Camp (FPC) Seymour Johnson, located in Goldsboro, North Carolina. Award will be made to the quoter who submits the most advantageous offer to the Government based on price and other factors. Faith-Based and Community-Based Organizations have the right to submit quotes equally with other organizations for contracts for which they are eligible. General Requirements: The Federal Bureau of Prisons (BOP), Federal Prison Camp (FPC) Seymour Johnson AFB , located at 1055 Peterson Avenue, Goldsboro, NC 27533, has a requirement for the supply of bag in the box (BIB) beverage products and equipment for a one year base period with one 12 month option period. Award is estimated to be on or about September 8, 2004, with the effective award date estimated to be October 1, 2004. The contractor shall provide and install 2 refrigerated dispensing units with at least 4 heads each and two 150 lb. Ice Bins (Hopper) in the FPC Seymour Johnson inmate dining room. Contractor shall provide and install BIB beverage products and equipment necessary for dispensing the product (i.e. carbonation units, storage racks, syrup pumps, chillers, gauges, flex lines, fittings, and all required attachments) in a manner which is consistent with commercial industry standards and regulations. The government may request additional equipment at no additional charge from the contractor at any time throughout the life of the contract in order to meet the increased demands of the institution due to population increases or remove equipment as necessary. All equipment provided shall remain the property of the contractor and shall be removed at the contractor's expense upon completion of the resulting contract. Contractor shall install and have operational required equipment within two weeks of contract award. Installation will be coordinated between the FPC Seymour Johnson Food Service Administrator and the contractor. The contractor shall service and maintain all provided equipment . The Food Service Administrator will notify the contractor of any problems with the beverage equipment. Response time for repairs shall be within 24 hours from notification to the contractor. A quick response time is critical due to the nature of the Food Service operation within the correctional facility. The contractor shall provide CO2 gas propellent (at no additional cost) , if applicable, to dispense product from the beverage dispensing units. Contractor shall exchange empty CO2 gas bottles for full units upon delivery of product or as needed. Security Procedures: Contractor service vehicles may be searched upon entering and departing Air Force Base and/or institution grounds. Contractor personnel must present proper personal identification and register their vehicles at the Base Visitor?s Center. The delay for such procedures is minimal under normal circumstances. In the event of unusually severe weather, institution or military emergencies, or any other unusual circumstances, the delay may exceed ? hour which could result in the vehicle not being permitted to enter the base/institution grounds until further notified. Contractor service personnel are required to pass security/investigative clearances prior to entering the institution. The following investigative procedures may be applied and appropriate forms completed before an individual is permitted inside an institution: National Crime Information Center (NCIC) Check Name Check FD-258 Fingerprint Check Law Enforcement Agency Checks Vouchering of employers over the past five years Completed OF-306, Declaration for Federal Employment, and Appropriate Resume or Optional Application Completed Contractor Pre-employment Form Release of Information Urinalysis Delivery Order Information: Delivery orders for required products will be placed telephonically by an institution Contracting Officer or authorized food service staff under the terms and conditions of the contract. An order number will be given to the contractor at the time the order is placed. All contractor invoices shall reference the applicable delivery order number. All contractor deliveries shall be to the FPC Food Service Warehouse between the hours of 8:00 a.m. and 3:00 p.m., local time, Monday thru Friday, excluding Federal holidays. Deliveries are to be shipped complete unless otherwise agreed upon by the institution Contracting Officer. All items shall be delivered F.O.B. destination. Products: Contractor shall provide 5 gallon and or 2.5 gallon non-alcoholic, non-carbonated, liquid concentrate, bag in box (BIB) beverages of natural or equal flavoring. BIB ratios may vary. However, price will be evaluated on the final product price. Contractor shall provide the following flavors and estimated quantities: sweet tea (approximately 15, 000 gallons of final produce per year) unsweet tea (approximately 5,000 gallons of final produce per year) fruit flavor sweet (approximately 15,000 gallons of final produce per year) artificially sweet fruit flavor (approximately 5,000 gallons of final produce per year) Quantities listed are estimates only and are not a representation to an offeror or contractor that the estimated quantities will be required or ordered or that conditions affecting requirements will be stable or normal. Pursuant to FAR 17.203(b), the Government?s evaluation shall be inclusive of options. **Award will be made on an ?All or None? basis. ?All or None? meaning only contractors who bids on all items listed in the schedule will be considered for award.** Special Requirements All machines furnished hereunder shall be equipped with all necessary safety devices listed by Underwriter?s Laboratories Inc., and be equipped so as to provide thermal overload protection. All product dispensing machines furnished must be approved by the National Sanitation Foundation or other approved testing organization. The contractor assumes full risk of and responsibility for any loss, destruction or damage occurring to the dispensing machines except such loss or damage as may be attributable to the Bureau of Prisons by reason of negligence of its employees while acting within the scope of their employment. Schedule of Required Insurance: As required by FAR clause 52.228-5 entitled Insurance - Work on a Government Installation, the contractor shall, at its own expense, provide and maintain during the entire performance of the resulting contract, at least the kinds and minimum amounts of insurance as follows: Workers Compensation: the contractor shall comply with applicable Federal and State workers compensation and occupational disease statutes - if occupational diseases are not compensable under those statutes, employer?s liability coverage of at least $100,000 is required; Comprehensive General Liability $500,000 per occurrence; Automobile Liability: $200,000 per person, $500,000 per occurrence for bodily injury, and $20,000 per occurrence for property damage. Evaluation of Quotation The Government will make award based on technical compliance (including taste acceptability), price and past performance. Technical compliance and past performance when combined are approximately equal in value to price. Past Performance The quoter shall include a list of the three most recent current or complete contracts and subcontracts occurring during the past three years for the provision of similar beverage products. Contracts listed may include those entered into with the Federal Government, agencies of state and local government, and commercial entities. The quoter may provide information on problems encountered on the contracts and subcontracts identified and may describe corrective actions taken to resolve those problems. Past performance subfactors to be evaluated are: quality of product or service; timeliness of performance; business relations; and customer satisfaction. Submission of Quotations Quotations shall be submitted on or before the quotation deadline specified below. All responsible sources may submit a quotation. Quoters will be required to submit the following information on letterhead or business stationery, directly to the contracting officer. 1. Completed Schedule of Items 2. Offerors Representations and Certifications (FAR 52.212-3) 3. A clear description of the product(s) offered, to include identification of brand and product name. Product samples may be requested. 4. A list of the three most recent contracts awarded to the quoter or a major subcontractor, to include a point of contact, address, and phone number, from whom the Government will seek past performance information. 5. Offerors are required to submit product samples in order to determine product quality and taste acceptablity. All quoters are advised that this solicitation includes the clause at FAR 52.212-4 for commercial items which at paragraph (t), Central Contractor Registration (CCR), requires all contractors doing business with the Federal Government after September 30, 2003, to be registered in the CCR database. Quoters should include their Data Universal Number System (DUNS) number in their quotes. The Contracting Officer will verify registration in the CCR database prior to award by entering the potential awardee?s DUNS number into the CCR database.
