Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation FA4877-05-Q0054 is issued as a request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. (iv) This procurement is being issued as a 100% SMALL BUSINESS set-aside under NAICS code 713940 and small business size standard $6,000,000. (v) Provide Aerobic Instruction for a base period of 01 Oct 2004 through 31 Sep 2005, to include four (4) one (1) year option periods. (vi) Contractor shall provide instruction in toning and stretching for 2 days per week and 2 days of cardio exercise classes, i.e. Low Impact and Circuit training for a total of 4 days per week. Classes will be for a maximum of one (1) hour, which shall include 50 minutes of exercise; 5 minutes of warm-up, and 5 minutes of cool down session. Contractor must be able to deviate one (1) hour during winter/summer work schedule change, In accordance with Statement of Work.
