- Description
- The purpose of this notification is to provide answers to additional questions received to date regarding solicitation 200-2004-Q-01613. Question 1: What is the total number of samples that CDC expects to submit to the laboratory? Answer 1: 123 samples for nicotine, moisture, and pH analysis (41 samples done in triplicate). 123 samples for tobacco specific nitrosamine analysis (41 samples done in triplicate). 39 samples for metals analysis (13 samples done in triplicate). 13 samples for pesticide analysis. Question 2: Will these samples be coming to the lab in one shipment or separate batches? If in batches, what would be the typical size of each batch and the shipment frequency? Answer 2: These samples should be coming to the lab in one shipment. Question 3: What is the expected turn-around time for sample analysis or for project completion? Answer 3: Six (6) months after the samples are received by the contractor. Question 4: Are there any specific reporting requirements (e.g., required formats for QC data or tables or documentation), and if so, can you provide additional information on these requirements? Answer 4: We request that copies of standard curves and QA/QC documentation be provided. The format can be at the discretion of the contractor. Question 5: Are we to be submitting reports as the work is completed or a summary report at the end of the project? Answer 5: One report when the work is completed. Interim reporting is not required. Question 6: Would you require an electronic copy of the report, a hardcopy of the report, or both? And if an electronic copy is required, do you have a preference with regard to the format (i.e., Word, WordPerfect, and so on)? Answer 6: Electronic and hardcopy. Electronic copy should be in Word format. Question 7: Does our organization need to represent and certify 52.212-3(b)(3) Taxpayer Identification Number? Answer 7: Yes. Question 8: Is 52.212-3(g)(2) or (3) Buy American Act-Free Trade Agreements-Israeli Trade Act Certificate applicable? The solicitation does not specifically state this requirement. Answer 8: No. The Buy American Act-Free Trade Agreements-Israeli Trade Act Certificate is only applicable to acquisitions for supplies, or for services involving the furnishing of supplies, for use within the United States. Since this acquisition does not require the vendor to furnish supplies to the CDC, then this certificate is not required. Question 9: Is 52.212-3(h) Certification Regarding Debarment, Suspension or Ineligibility for Award applicable? Does the contract value exceed the simplified acquisition threshold?
