- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals ar e being requested and a written solicitation will not be issued. REVISED SOLICITATION. Contractor is to provide a revised proposal on number and type of surveyors required, estimated number of days, report expenses, travel expenses, proposed survey, dates that surveys can be done, and any other expense. Contractor will conduct a pre-survey assessment visit (PSAV) of all applicable services/departments within th e Medical Center and Outlying Clincs (Joel, Clark, and Robinson Health Clinics) prior to the survey conducted by the Joint Commission on the Accreditation of Healthcare Organizations (JAHCO). Primary dates of survey: September and October, Secondary dates : 1st and 2nd weeks of November and 2nd and 3rd weeks of December. The visit will be conducted to assess compliance with established standards reviewed during a scheduled survey or a random unannounced survey. The PSAV will provide consultation, education, analysis and evaluation to assist the organization in identifying areas of strengths, areas for improvement and recommendations on how to meet the intent of the regulatory standards. The PSAV will include the following: a. Review of pertinent information and documents. This review will occur either in advance of the survey or on the first day of the survey. Some documents may be requested from the organization for review in a dvance of the on-site survey. A list of required documents will be provided upon acceptance of this proposal. If the review occurs in advance of the survey, the organization will be expected to submit these documents to the contractor one month prior to the PSAV. b. Interviews with organizational leaders and department directors to review information, procedures and policies, and documents to assess compliance with the standards. c. Patient unit visits and patient interviews to assess compliance with the standards, safety, infection control and patient education. d. Discussion with medical staff and department chairpersons, as applicable, to review and assess programs and activities, including an evaluation of the quality and appropriateness of patient c are and privileging and credentialing for licensed independent practitioners and allied health care staff who are credentialed through the medical staff process. e. An assessment of the organizationâs Performance Improvement Program and department/se rvices, sentinel event, ORYX quality control processes and outcomes. f. An assessment of the organization-wide Information Management Plan and the priorities to meet internal and external information needs. g. Individual and group education to provide an understanding of the standards and intents, and to identify best practices for achieving compliance. h. Daily briefings and an exit conference with the facility leaders to identify non-compliance to standards and recommendations for improvement. i. An opt ional medical staff leadership conference to review areas of compliance and recommendations for improvement. j. Key focused areas such as those found in random unannounced surveys will be reviewed. k. Conduct individual and system tracers, using current JCAHO tracer methodology. Consultation will be provided by a primary team of professionals to include a physician, a nurse, a behavioral health professiona,l, and someone who can look at clinical support services such as Information Management, lab, pharma cy, orientation, and human resources. Consultants shall be former employees of the Joint Commission on the Accreditation of Healthcare Organizations as either part-time or intermittent surveyors or shall have the same knowledge and exper tise as those employed by the Joint Commission on the Accreditation of Healthcare Organizations. Additional surveyors may be added if there is a need for a tailored component. A written report will be generated and forwarded to the healthcare facility wit hin 30 business days of PSAV completion. The report will be available in electronic version. A hard copy may be requested by the organization. This report will offer findings keyed to regulatory standards and provide recommendations on achieving complia nce. In addition, relevant sample material, policies and procedures will be available for resource. The consultants will be available to assist the facility with preparation of the periodic performance review (PPR) and any responses or rebuttal reports t o regulatory agencies. The team leader consultant will be available by telephone or email to assist with any post-visit questions. Future on-site consultation visits may be requested to assess progress made, provide coaching and tailored training as requir ed. Payment for these visits will be negotiated under a separate contract agreement. The contractor is responsible to cover the travel expenses. Government will reimburse allocable and allowable costs in accordance with the Joint Federal Travel Regulatio n. This is a one time contract. NAICS CODE 546111, SIC CODE 8049, FSC U099.
