Description
MODIFICATION 6: The purpose of this modification is to state the following: 1) Offerors shall fax the first two pages of the solicitation only by the deadline. Page 1 of the solicitation shall be signed in Box 30a by an authorized representative of the company; Box 30b shall contain the signers printed name and title and Box 30c shall contain the date. The second page shall contain the amount the offerors are quoting. Offerors shall be ready to fax the remainder of the solicitation (i.e. SF1449 immediately upon being contacted) to include the Offerors Reps and Certs (to be filled out completely) beginning on pages 17 through 22. All small disadvantaged businesses shall fax proof of their status with the first two pages of their SF1449. 2) Offers shall send their proposal to one of the following fax numbers (703) 875-5390; (703) 875-7370; or (703) 875-6105. Offerors shall call Dept. of State P.O.C., Kevin T. McGhee, when they have faxed their proposal to confirm receipt. The U.S. Department of State (DOS) Office of Logistics Management, on behalf of the Office of Overseas Building Operations, Operations Management, Division of Art in Embassies anticipates awarding a 12 month, All Risk (excluding War and Terrorism coverage) insurance contract. The contract shall insure $50,000,000 worth of art, properties or items displayed at (and in transit to and from) Department of State properties worldwide. The art, properties or items insured under this contract shall be insured at all times from the commencement date of the contract until expiration. This requirement will be a full and open competition with a ten percent evaluation preference going to all small disadvantaged businesses in accordance with FAR Clause 52.219-23. This is not a request for proposal. The Government intends to release the Request for Proposal on or about July 22, 2004 as a modification to this Synopsis.
