Description
This FedBizOpps announcement is notice that TRICARE Regions Southwest & Central (6, 7 & 8) which includes all Military Medical Treatment Facilities (MTFs) in the states of Texas, Arkansas, Louisiana, Colorado, Arizona, Kansas, Missouri, Idaho, Oklaho ma, New Mexico, North Dakota, Montana, Nebraska, Nevada, South Dakota, Utah, and Wyoming are initiating the standardization process for the product line of Pharmacy Labels used for general prescription requirements. GENERAL REQUIREMENTS are as follows: 1) Offers labels, including warning labels. 2) Labels that are sheets available in 8 1/2 inch X 11 inch and 8 1/2 inch X 14 inch sheets. 3) Label sheets compatible with laser printing. 4) Label sheets that do not jam laser printers during high volume processing. 5). Label sheets that are perforated to accommodate prescription labeling requirements and patient medical information (PMI). 6). Label sheets that are perforated when required to accommodate specialty-warning labels. 7). Label sheets that operate with current DoD specifications and laser pri nter configurations. 8). Label sheets that are smear and fade resistant. 9). Label sheets that lay flat for proper printer feeding. 10). Labels are easily removed and applied. 11). PMI that is easily removed from adhesive labels. 12). Packaging that preven ts damage while stored in uncontrolled environment. 13). Must carry a full product line of labels with various applications/styles to meet the needs of each MTF. 14.) Submitter must provide customer support in the form conversion where applicable, clinical support, education programs, utilization reports and sales representative for each MTF within TRICARE regions 6, 7, & 8. SPECIFIC REQUIREMENTS: 1) Are your products available through DoD Pharmacy Prime Vendors (PVs) or Medical-Surgical PVs: Cardinal Health, Bergen, and Dakota Drug? If yes, what are your DAPA Numbers For information on the Medical-Surgical DAPA process, contact Mr. John Charalabidis at (215) 737-7124. For information on the Pharmacy DAPA, contact Major John Howe at (215) 737-3262. 2) Under what brand (s) does your company manufacture or distribute your products? 3) Is there a history of back-orders or recalls of your label product(s)? If yes, please answer: a) Dates and duration, b) Cause, c) Resolution. This is not a Request for proposal, and samples are not being requested at this time. The intent of this standardization is to establish regionally p referred sources for hospital supply items to standardize purchasing of products for regional MTFs. In exchange for becoming a preferred source for TRICARE Southwest & Central Regions, the government requests additional price considerations for these produ cts. Additional TRICARE Regions may elect to join after award and additional price discount consideration will be requested at that time. Clinical Product Teams (CPTs) will evaluate this product line. CPTs are composed of healthcare providers who have expe rtise and knowledge of the products. They will review products and literature submitted. TRICARE Regions Southwest & Central deem the CPT members to be medical experts. Therefore, they are qualified to determine and recommend which products are acceptable. Their recommendation can be accomplished by literature review, presentation, and demonstration, physical or clinical evaluation. Respondents must address all criteria listed above. Also include a cover letter identifying a point of contact to address questions regarding your p roducts. Companies not responding by the above date may be eliminated from consideration on any subsequent procurement action for these items. All responses to this notice must be received by close of business 9 September 2004.
