Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F1303341910100, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24 and Defense DCN 20040625. The solicitation listed here is 100% small business set aside. The associated Standard Industrial Classification (SIC) is 7378. The North American Industry Classification System is 811212 and the associated business size standard is $21,000,000. This request for quotation consists of the following: Item 0001: One year full support maintenance agreement for the RTAS Digital Dictation System for David Grant Medical Center, Travis AFB, CA, Period of performance from 1 Oct 04- 30 Sep 05. This includes parts, labor and travel to and from Travis AFB. Equipment list with cost breakdown is required with submission. The following provisions and or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. FAR 52.212-2 Evaluation-Commercial Items, paragraph (a) to this provision is completed as follows: Price Only. FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for an award. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (b)(5), (14), (15), (16), (17), (18), (19), (20), (30) and FAR 52.204-7, 52.209-6, 52.247-34, 52.253-1, DFAR 252.212-7001 Contract Terms and Conditions (Dev), 252.204-7004 (Alt. 1), 252.232-7003, DFARS 252.225-7001 Buy American Act and Balance of Payments Program. DFARS 252.225-7002 Qualifying country sources as subcontractors. FAR 52.252-2 52.252-6. Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
