Description

The government intends to negotiate a Firm Fixed Price type contract for the Proficiency Testing Program (PT) which leads to the laboratory accreditation and proficiency ratings approved by the American industrial Hygiene Association (AIHA) as required by Navy instructions and the Navy Environmental health Center, Portsmouth, VA. Associated with the accreditation programs are annual accreditation and PAT program fees for the industrial hygiene PAT and the Environmental lead PAT. The contractor shall provide Proficiency Analytical testing (PAT) samples of Metal, organic solvents, Silica, asbestos fibers, and formaldehyde on a quarterly basis to one lab (lab #101695) Asbestos types shall be chrysotile and amosite asbestos. The metal shall be lead, cadmium, Zince and Chromium. The solvents shall be common industrial solvents selected by AIHA. The samples shall be submitted as specified. The contractor shall have the necessary personnel, materials facilities and technical services required to provide and maintain a proficiency testing quality assurance program for a maximum of thirty four (34) Navy Testing Laboratories. The resulting contract will be from the date of award to 30 September 2005 with four one-year options. The standard industrial (SIC) is 8734. Offeror is hereby notified that the solicitation and subsequent amendments to the solicitation will ONLY be available by downloading the documents at FISC Norfolk Website: http:// www.nor.fisc.navy.mil or http://www.neco.navy.mil. By submitting a proposal, the vendor will be self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any federal Agency. Failure to so represent may be grounds for refusing to accept the proposal. Number note 22 applies.
