- Description
- The Contractor shall provide all emergency, urgent and other requested ground ambulance services for Kayenta Health Center, Kayenta, AZ and Inscription House Health Center, Inscription House, AZ. Services shall be available 24 hours per day seven days a week. A fixed monthly amount shall be compensated to the Contractor for ground ambulance services provided in accordance with the statement of work. The fixed monthly amount shall include base rate, mileage rate, medical, supplies, oxygen, personal protective equipment, patient care equipment, linens, personnel and all necessary services. The closing date for receipt of proposal is August 13, 2004, 4:30 PM (local time). A single award will be made for twelve month period.
