- 04R6091- KVM Switch, User Stations, Parts and Installation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number is FA8604-04-R-6091. This solicitation and incorporated clauses/provisions are those in effect through Federal Acquisition Circular 2001-24 effective 19 July 2004 and Class Deviation 2004-o0001 and DFARS Change Notice 20040625. The solicitation incorporates by reference the following clauses: FAR 52.204-7, 52.212-1, 52.212-3, 52.212-4, 52.212-5, and DFARS 252.204-7004 Alternate A. NAIC Code: 334290. This requirement is to procure KVM Switch, User Stations, parts and installation as follows: Line Item Description Model Number Qty 0001 Paragon 4 user x 42 systems UMT442-RK 1 unit KVM Switch Cat5 Cable Based Matrix Switch 0002 User Stations for Paragon 4 UST1 4 unit 0003 Computer Interface Module (CIM) UKVMC 25 unit for Standard PS/2 Connected Devices Includes Local Ports for Local Keyboard, Monitor, and Mouse 0004 Installation shall include: (1) removal 1 Lot of old KVM equipment, (2) replacing and installation with the of above line items 0001 ? 0003, (3) Cat5 Cable, (4) running of cable, (5) installing system and, (6) providing training. Offerors must quote all items or none to be considered responsive. FOB Destination. The Ship To address is Wright-Patterson AFB OH 45433-7320.
