Description
(1)This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6. included in the notice in accordance with 38 U.S.C 8153, Sharing of Health Care Resources with non-VA entities. This announcement constitutes the only solicitation. Proposals are being requested and written solicitations will not be issued. (2) This solicitation is issued as RFQ 573-62-04. (3) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 2001-24. (4) This solicitation is advertised as a small business set aside with NAICS: 561310 with a size standard of $5 million. The procurement shall be conducted under FAR 13 simplified acquisition procedures and FAR 12, Commercial Items . The Contractor shall furnish all necessary supervision and qualified competent personnel to perform Biomedical Technician Services for the Department of Veterans Affairs, North Florida South Georgia Health System and at the VA Medical Center, Gainesville, FL . [Line Item One]: 1 each 1.0 Full time Biomedical Technician (1.0 FTE) for placement at the VA Medical Center, Gainesville, FL. Services to consist of installing, inspecting, testing, evaluating, modifying and maintaining biomedical equipment and biomedical systems. The incumbent?s general responsibility is to ensure quality performance of diagnostic and therapeutic biomedical systems with maximum safety to the patient and the operator. The incumbent will primarily perform the corrective maintenance of highly complex biomedical systems which require comprehension and skill in the disciplines of anatomy, physiology, biology, chemistry and medicine, as well as, many fields of engineering including electrical, electronics, mechanical, pneumatics, hydraulics, thermodynamics and optics. Incumbent performs duties of broad scope and complexity to install, repair, test, calibrate, adjust, overhaul and correct system malfunctions with a variety of medical, surgical, x-ray and other equipment, ranging from gas flow meters to complex medical specialty systems that may have interrelated and dispersed sub-systems. Maintains high standards of equipment performance and safety to protect patients and the medical staff. Inspects and evaluates new systems upon receipt to determine that all safety regulations, manufacturer specifications and contract requirements are met. Performs operational check on new equipment and after repairs have been completed to insure equipment is operating within prescribed limits. Performs scheduled and unscheduled preventive maintenance to assure operational safety of equipment and maintains appropriate maintenance records. Serves as a technical advisor to clinical staff in the purchase of new equipment. Writes technical reports on projects for submittal to the supervisor. Instructs users in the proper use of equipment and procedures related to safety and operator maintenance. Responsible for parts acquisitions. Subject to emergency call 24 hours per day. Work hours are 40 hours per week, 8:00am - 4:30pm Monday through Friday. On call services are required for Holidays, Weekends, and Monday through Friday from 4:31pm to 7:59am. Contractor?s employee must be present at the VA Medical Center and perform services on site for the period specified in the contract. Payment shall be made in arrears. Payment for annual leave, sick leave, holidays, etc. are the responsibility of the contractor. The Contractor shall comply with JCAHO requirements and maintain competence assessments for the contract personnel selected to perform services for this contract. Professional liability and worker?s compensation insurances are required of the Contractor. Contracted personnel selected for this assignment shall attend a VA Orientation. The following FAR clauses and provisions are added by addendum to FAR 52.212-4; 52.204-7 Central Contractor Registration, 52.217-8 Option to extend Services; 52.224-1 Privacy Act Notification; 52.224-2 Privacy Act; 52.228-5 Insurance Work on a Government Installation, 52.232-18 Availability of Funds, 52.232-19 Availability of Funds Next Fiscal Year, 52.237-3 Continuity of Services, 52.216-18 Ordering, 52.216-19 Order Limitations; The following VAAR clauses and provisions are added by addendum: 852.270-1 Representatives of Contracting Officers; 852.270-4 Commercial Advertising. FAR provision 52.212-1 Instructions to Offerors - Commercial is hereby include. The following is an addendum to FAR clause 52.212-1; Offerors are advised that proposals may be evaluated without discussions. Award will be based on price, past performance and technical capability. Technical capability and past performance, when combined, are more important than price. Offerors are requested to submit documentation to support the technical capability and past performance of their company and the individual that is referred for placement to perform services. To be responsive to the solicitation offerors shall submit candidate credentials. The VA intends to award a single contract and intends to make an award based on the best value by performing a price/technical tradeoff analysis. In accordance with FAR subpart 9.104-1, each offeror is required to provide information on a reasonable number of recent and relevant contracts for the same or similar services, as well as the offerors primary financial institution, to include contract numbers, verifiable points of contact, telephone numbers and any other relevant information which will be used by the Contracting Officer to make a responsibility determination about a prospective offeror. FAR provision 52.212-3 Offeror Representation and Certification shall be completed and submitted with the proposal. The contract base period will be effective from date of award on or about September 13, 2004 through December 3, 2004. Total duration of the contract will not exceed 90 calendar days. The closing date for all proposals is September 8, 2004 at 3:00pm local time.
