Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued, paper copies of this solicitation will not be available. This is a commercial solicitation using Simplified Acquisition Procedures found in FAR Part 13, soliciatation no. FA4690-04-R-C001 and is issued as a request for quotes (RFQ) on NLT 10 Sep 2004. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2001-24. FAR clause descriptions are available via the Internet at http://farsite.hill.af.mil/. The set-aside for this solicitation is 100% small business. NAICS code: 336211. Size Standard: 1000 employees. This solicitation is to ?lease to buy? a P26 Water Tanker Vehicle, NSN 4210013564907, or equivalent. Truck completion date will be within 9 months after signing contract. Collect calls will not be accepted, e-mail: carl.lynch@ellsworth.af.mil. STATEMENT OF WORK FOR US AIR FORCE WATER TENDER VEHICLE (P-26 EQUIVILENT) ONE (1) TOTAL SECTION 1. GENERAL 1.1 SCOPE OF WORK. Ellsworth Air Force Base, using the AFCAP (Contractor) shall provide safe operational NEW vehicles and service in accordance with this contract. 1.2 POINTS OF CONTACT. Contractor will provide points of contact within their company to interface with contract representative and Fire Departments Quality Assurance Inspector (QAE) on issues concerning leased vehicles. Names, business address, phone numbers, and hours of operation are the minimum information requirements. Contractor shall provide an after hours name and telephone number for emergency contact. SECTION 2. DEFINITIONS 2.1 TECHNICAL DEFINITIONS PARTICULAR TO THIS STATEMENT OF WORK 2.1.2 Acceptance inspection-An inspection shall be performed on all vehicle and equipment prior to placing them in-service. This inspection shows the overall condition of the vehicle. Discrepancies must be corrected prior to placing the vehicle in service if use of the vehicle would aggravate the problem or if the discrepancy creates a safety hazard. 2.1.2 Accident repairs-Repairs required as a result of collision; regardless of whether the object collided with a vehicle, or whether the vehicle requiring repairs was the moving unit. 2.1.3 Criminal act-A violation of civilian criminal law. 2.1.4 Disabled vehicle-A damaged, worn out, or malfunctioning vehicle. 2.1.5 Fair wear and tear-The deterioration of items attributed to normal usage. 2.1.6 Hostile act-An act of war or other armed conflict. 2.1.7 Scheduled maintenance-Periodic prescribed inspections or servicing of equipment, accomplished on a calendar, mileage, or hours-of-operation basis. 2.1.8 Unscheduled maintenance-Maintenance that is not scheduled but is required to correct deficiencies and to restore the vehicle or equipment to a serviceable condition. SECTION 3 GOVERNMENT FURNISHED ITEMS AND SERVICES 3.1 Fuel-Government is responsible for supplying fuel used in leased vehicles after acceptance. 3.2 Joint Inspection-Contractor?s representative, USAF fire protection representative, and QAE must perform a joint inspection and document in writing, vehicle condition upon delivery and redelivery. SECTION 4 CONTRACTOR FURNISHED ITEMS AND SERVICES 4.1 General-Contractor shall provide specified vehicles and all services required to keep vehicles in a safe, operational condition. Vehicles will be delivered within the timeframe agreed to in the contract and must meet minimum acceptance criteria provided herein. The time frame for delivery will be as follows. The vehicle is required to be in place within 90-days of acceptance of contract. Failure to pass acceptance inspection in time frame specified will result in damages, both actual and/or liquidated, being paid by the contractor to the U.S. Government. Contactor will furnish two (2) sets of Operation Manuals, one (1) full set of maintenance manuals and one (1) Service Parts listing to include an illustrated parts breakdown for each vehicle. All manuals shall be in English. Also, at the end of the contract the manuals will become property of U.S. Government. The U.S. Government reserves the right to return vehicles prior to expiration of full lease period without penalty. In the event the vehicles are returned, the U.S. Government will not be held responsible for any remaining lease payments. 4.1.1 Delivery of vehicle-Contractor shall deliver the vehicle to the base vehicle transportation yard of the location identified by U.S. Government representative on date specified in the contract and by terms of the contract. Delivery shall be accomplished with coordination made with the Quality Assurance Evaluator (QAE). The QAE will be the Fire Chief at each location. Delivery shall be accomplished during normal business hours of 8:00 AM to 5:00 PM, unless requested otherwise by the QAE. 4.1.2 Insurance-Contractor will carry insurance to cover cost of vehicle replacement or repair regardless of incidence requiring replacement or repair. This is to preclude the government from being held liable for claims generating from any of the above. 4.1.3 Records-Contractor shall maintain records of transactions with the U.S. Government and, upon request, make such records available to the QAE or designated representatives within 72-hours. 4.1.4 Vehicle-The following minimum criteria shall be met at the time of acceptance inspection: 4.2.1 Condition-Both exterior and interior of vehicle must be free of soil, rust, and damage during initial vehicle acceptance inspection. Condition following scheduled or unscheduled maintenance will be at the direction of QAE or designated acceptance individual. Contracting officer (CO) has final determination authority if differences of opinion arise between contractor and designated accepting individual. 4.2.2 Safety items-Each vehicle must meet all host nation and U.S. Government safety requirements and have, as a minimum, the following safety features: Head lights, warning beacons (visible on four (4) sides of the vehicle), seat belts, rear view mirrors, windshield wipers, parking brake, and horn. 4.2.3 Heating and Air Conditioning. Both heating and air conditioning are required in all vehicles. 4.3 Services. The contractor shall provide the following services: 4.3.1 Maintenance-Contractor shall support the Air Force maintenance officer when necessary to locate/source vehicle parts and components to facilitate rapid repairs. 4.3.2 Contractor-Will respond to QAE by phone no later than 24-hours after QAE has contacted contractor for repair actions. 4.3.3 Scheduled Maintenance-QAE will maintain the maintenance schedule for warranty requirements. The QAE will notify contractor when warranty maintenance/repair is due. The QAE will provide a facility located at each installation where maintenance/repair can be performed. The QAE is responsible for all preparation needed to allow the contractor access to the installation and provided facility (i.e. access, escorts, search, exit passes, etc.) if necessary. Contractor will provide all needed tools to conduct the warranty maintenance/repair, or shall support AF maintenance personnel with parts/services, on each vehicle at al location. Contractor will dispose of all debris and waste generated/accumulated during warranty maintenance. 4.3.4 Repairs-Repairs are separated into two categories, major and minor repairs. Minor repairs; shall not exceed ?2-hours?. Major repairs; will be performed at a designated facility identified by the QAE. Contractor is responsible for movement of vehicle beyond the installation gate if warranty work of major repairs cannot be accomplished at the installation. Major repairs shall not exceed ten (10) working days to include procurement of part(s). NOTE: The contractor, if necessary, will use the most expeditious means of ordering replacement parts (i.e. premium shipment) and making required warranty repairs if necessary to ensure vehicle is out of service for the least amount of time possible. The QAE is responsible for all preparation needed to allow the contractor access to each installation and provided facility (i.e. access, escorts, search, exit passes, etc.) for any warranty work requirements. Contractor will provide all needed tools to conduct the warranty work, or shall provide all necessary parts/services required to accomplish warranty work at each installation. Contractor will dispose of all debris and waste generated and accumulated during warranty work. See attachments 2 and 3 as general guidance for defining minor and major maintenance. These lists do not constitute a requirement for maintenance to be a part of this contract. 4.3.5 Repairs over 30-days- Acceptability are at QAE?s discretion (Warranty only). 4.3.6 Maintenance as a Result of Negligence/Abuse-Intentionally left blank. 4.3.7 Vehicle Requirements (General). Contractor shall provide new vehicles with less than 1,500 miles on the odometer. Vehicles will be in excellent physical condition and mechanically sound, with no visible leaks, damage or signs of wear. Retread tires are unacceptable. Any tires determined by QAE as not meeting these specifications will be rejected and contractor will be required to replace tires at no additional cost to the U.S. Government. All vehicles shall be registered, licensed, and insured for the full coverage required under the laws of the state/country for which the vehicles will be operated. 4.3.8 Vehicle Requirements (Specific)-The following paragraphs outline the specific details for the Hazardous Materials Response Vehicle (P-31) to meet minimum Air Force mission requirements. Attachment 1 identifies specific vehicle requirements. 4.3.8.1 The offer or/contractor is encouraged to use recovered materials in accordance with paragraph 23.403 of the Federal Acquisition Regulation (FAR). 4.3.9 For the purpose of this requirement, recovered materials are those materials that have been collected from sold waste and reposed to become a source of raw materials, as distinguished from virgin raw materials. The components, pieces and parts incorporated in the vehicle may be newly fabricated from recovered materials to the maximum extent practicable, provided the vehicle produced meets all other requirements of this commercial item description (CID). Used, rebuilt or re-manufactured components, pieces and parts shall be incorporated. 4.4 Contractor will provide advanced maintenance training to vehicle maintenance personnel on location, at the bases where the vehicles are assigned. Subject training should be sufficiently in-depth to qualify USAF mechanics in major and minor maintenance aspects of the vehicle. Training will be provided NET 90-days prior to contract expiration, and NLT 30-days prior. 5. QUALITY ASSURANCE PROVISIONS 5.1 Product conformance. The products shall meet the salient characteristics of this commercial item description, conform to the producer?s own drawings, specifications, standards, and quality assurance practices, and be the same product offered for sale in the commercial market. The government reserves the right to require proof of such conformance. 5.2 Commercial Item Description. The vehicles furnished must meet the ?commercial item? definition, as specified in FAR 2.101, as of the date of award. The government reserves the right to ask the offer or/contractor to prove that their product meets the referenced commerciality requirements. The offer or/contractor shall provide two copies of their commercial descriptive catalogs with their offer, as specified in Clause I106. The offer or/contractor shall identify all modifications made to their commercial model in order to meet the performance and descriptive requirements of the CID or referenced documents. In regard to the offered item, the offer or/contractor shall identify any/all variations from compliance with or modifications to the performance requirements of the CID or the referenced documents. 5.3 Contractor requirements. The contractor shall provide two sets of operation, maintenance, and part?s manuals, in accordance with the requirements of the contract, with each vehicle delivered. The part?s manuals shall list the contractor?s part number for each component plus a manufacturer?s part number for each purchased component, as applicable. Manuals shall be provided for review 30-days prior to the time of demonstration. The contractor shall provide a product familiarization video tape with each truck, that gives a verbal and visual description of all the information required for operation and routine maintenance of the truck and its components, plus references to the appropriate portions of the respective manuals where more detailed information can be found. 6. PACKING. 6.1 Preservation and packaging shall be the minimum necessary to afford protection against corrosion, deterioration and physical damage during shipment from the supply source to the first receiving activity. 6.2 Unless otherwise provided in the contact, the truck shall be prepared for delivery by common carrier.
