Description
AMENDMENT: This combined synopsis/solicitation is hereby amended to include the following information: The size of the completed climbing wall should measure an approximate width of 15 meters (49.06 feet) and an approximate height of 6 meters (19.63 feet) for a total approximate area of 90m2 (962.85ft2). The approximate surface area should be 1203.56 Ft2. The completed wall should have a safe capacity of 10 climbers at any one time. This combined synopsis/solicitation is hereby extended until 10 Sept 2004. Quotes must be signed, dated and submitted by 10 Sept 2004, 0900 am, Japan Standard Time to the 35th Contracting Squadron/LGCB, Attn: Shane A. Horn, Contract Administrator, Commercial Phone 010-81-3117-66-3895, Fax 010-81-176-52-4793, E-mail shane.horn@misawa.af.mil. All other terms and conditions remain unchanged. COMBINED SYNOPSIS/SOLICITATION: This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ FA5205-04-Q-SH05. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2001-24 (as of 18 June 04). The NAICS Code 451110 applies to this solicitation. This RFQ contains nine (9) line items. Line Item 0001, Imprint Panels (manufacturer is Entre Prises USA) or Equal, Qty 90 EA, System Includes: Imprint Panels, pinch pipe substructure, standard belay anchors and fasteners. Vendor will supply a maintenance and owner's manual upon completion of the project. Imprint panel is the secondary supporting structure and does not include a channel rail, channel lock nuts, or primary support structure. Line Item 0002, Crack Panels w/Custom Steel (manufacturer is Entre Prises USA) or Equal, Qty 6 EA. Line Item 0003, Modular Holds (manufacturer is Entre Prises USA) or Equal, Qty 386 EA. Line Item 0004, Design/Engineering, Qty 1 EA, Vendor must use a registered engineering consultant to supply reaction forces at the top and bottom of the primary support structure columns and size the support columns. Design quote does not include local code review for climbing wall substructure or surface system. Line Item 0005, Shipping/Handling to Tracy, CA, Qty 1 LS (Lump Sum). Offeror must provide the estimated total weight and the total cubic ft (failure to provide this information may make offeror ineligable for award). Line Item 0006, Installation, Qty 1 EA, Installation does not include installation of the primary support structure or channel rail. Line Item 0007, Landing Zone Materials, (manufacturer is Entre Prises USA) or Equal, Qty 600 SF (Square Feet), 2.25? Landing Zone Materials. Line Item 0008, Landing Zone Installation, Qty 1 LS (Lump Sum). Line Item 0009, Auto-Belay System, Qty 3 EA.
