Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.This solicitation, W91QF4-04-Q-0012 is issued as a Standard Form 1449 (SF1449), 100% Set-Aside for Small Business. The solicitation document and corporate provisions and clauses are those in effect through Federal Acquisition Circular 2001-08 and DFARS Change Notice 20020531. The associated NAICS code is 562998 and small business set-aside size standard applicable to this procurement is $6.0 Million. The Northern Region Contracting Center, Ft. Leavenworth, KS has a requirement for the United States Disciplinary Barracks (USDB) for removing grease and cleaning a 1500gallon grease trap at the dining facility. Service will be provided Tuesday and Thursday with entry between 0900-1100 or 1300-1 500 hrs. Additional information may be obtained by e-mail from marie.odonnell@leavenworth.army.mil. Offerors shall submit quotes based on scope of work given. The Contracting Officer will evaluate on the basis of information furnished by the offeror or i dentified in the offer and reasonably available to the Contracting Officer in accordance with FAR 52.215-1. The Government anticipates award of a Firm Fixed Price Contract. The offerors should submit with their quotation: (1) Past Performance and Price f or service to be performed, proposed unit cost , total unit cost; (2) Supporting Information for proposed functional, or service characteristics specified above; (3) Contractors Duns and Bradstreet (DUNS) number; (4) Contractors Cage Code; and (5) An aff irmation that it has an active registration on the Central Contractor Registration (CCR), which is at the website www.ccr.gov. Contractors must be registered in the Central Contractors Registration database (www.ccr.gov) prior to the award of any Government Contract. The scope of work i s as follows: SCOPE OF WORK GREASE TRAP SERVICES FOR THE USDB FORT LEAVENWORTH, KANSAS 1. SCOPE OF WORK: The scope of work for this contract includes removing grease and thoroughly cleaning the trap. The contractor shall have their truck at the West Gate of the USDB, every Tuesday and Thursday of the week for 52 weeks. The hours of entry are 0900-1100 or 13 00-1500. This facility is a maximum security military prison. The scope includes all labor, material and equipment needed to clean the grease traps in a legal manner. 2. INFORMATION TO BIDDERS: 2.1 Access to and work inside the USDB are subject to the attached restrictions: 2.2 The U. S. Disciplinary Barracks is a maximum-security military prison. Paramount consideration will be given to security and safety of all parties concerned. 2.3 A list of Contractor personnel and vehicles, to include types of vehicles, color, and license plates numbers must be submitted to the USDB, Physical Security. The access roster will be submitted to USDB Physical Security at least 10 working days before commencing, for the purpose of a security investigation for person nel permitted on USDB properties. A minimum of 10 working days notice is also required to add personnel or equipment to the work force. Likewise, notification of personnel removed from the work force is required upon initiation of that action. It should be noted that the security investigation might result in the denial of access to the USDB for those personnel considered a security risk. This will not negate the contactors responsibility to provide personnel that meet security requirements or to perform services required under the contract. Additionally, special handling or parking may be required for equipment that could be used to brea ch security when idle. 2.4 All contractor personnel, vehicles and materials will be subject to inspection and search upon entry and exit. Except for emergency vehicles (fire trucks and ambulances). Contractor personnel and vehicles will be given first priority in entering and departing the USDB. 2.5 All contractors and contractor person nel will be required to read and comply with the USDB Regulation 190-3, Standards of Conduct and sign USDB Form 5, Statement of Understanding Prior to beginning work. Signed USDB Form 5 must be attached to the access roster upon submittal to Physical Security in order to process individual contractors for clearance. Contractor personnel and vehicles will utilize the West Service Gate. Personnel will sign for USDB badges in and out register after identification and clearance has been ascertained by the gate Guard upon entering and departing the institution. The badge will be worn on the outer garment at all times. Personnel losing a badge are required to report its loss to the USDB Investigations and the USDB Physical Security Office immediately. 2.5 When vehicles entering or leaving the USDB are searched, all personnel will required to dismount, shut the engine off, and open all doors and compartments. 2.6 Vehicles will not be permitted to enter or depart the West Gate Between 11:00 am and 12:30 pm except those deemed necessary to complete on going work. 2.7 Privately owned vehicles will not be allowed inside the USDB. 2.8 No vehicle will have keys left in the ignition switch at any time The vehicle is unoccupied. All vehicles will be locked when unattended. Vehicles/equipment without key ignitions will be rendered inoperable when unattended or idle. 2.10 Once inside the walls of the USDB, contractor personnel shall travel only from the West Gate to the work site. They will remain in the work site, and if required, will be escorted to other areas. 2.11 Contractor personnel are not permitted to talk with prisoners, except as actual work may required. Contractor personnel shall not give or accept any items from prisoners at any time. If there are problems with prisoners, the contact workers will notify the Physical Security personnel. 2.12 The following items are contraband and will not be brought into the USDB properties: Guns and firearms of any type. Ammunition, explosives. Knives (except those required as part of the tools used for the Job). Hazardous and poisonous chemicals and gases. Narcotics and drugs, to include marijuana in any form. Intoxicant, alcoholic beverages. Unattended currency. Razor blades (except those included in toolkits.) Obscene printed material (i.e., nudity emphasizing at state of Sexual arousal, graphic of body orifices and portrayal of Explicit sexual acts.) Photographic devices/equipment, audio or video recording devices. Extra clothing not worn on the job. 2.13 All tools and equipment will be secured under lock and key at all times unless in actual use. 2.14 The Commandant, USDB, may remove and permanently bar contractor personnel from the United States Disciplinary Barracks properties for actions that the Commandant deems may threaten or compromise the security of the USDB. Not withstanding the removal and /or bar of such personnel, the contractor will still be expected to fully perform all aspects of the job in accordance with provisions of the contract. 2.15 The Commandant, USDB may grant exceptions to these instructions on a case by case basis. Exceptions to specific items above must be submitted to the Contracting Officer, Fort Leavenworth, three (3) weeks prior to the bid opening date. If the exception (s) receives the approval from the Commandant, USDB, all bidders in receipt of the invitation to bid will be so Notified before the bid opening date. Exceptions to these instructions after work commences will generally not be granted. However, should unanticipated circumstances require a request for exception, that request must be submitted in writing as soon as the circumstances become known and in no case less than three working days prior to the desired effective date of the exception.
