Description
The purpose of this amendment is to incorporate the following changes: 1. Add Site Visit: 7 September 2004 at 1PM all contractors should contact Mr. Ed Shorey at 301-836-7198. 2. Section L  Instructions, Conditions and Notices to Offerors - NOTICE TO OFFERORS - Proposal Preparation Instructions: C. Special Instructions: & 1. Part I  Technical/Management Proposal shall include the following: a. Relevant Experience  The offeror shall provide information to demonstrate the depth, breadth, relevance and currency of demonstrated experience and technical expertise for performance on this contract. The offeror shall provide projects that are consi dered similar in type and scope. The offeror shall provide information on five projects that are current or have been completed within the last five years. The following information shall be provided: (1) Contract Number/Title/Location/Award Date (2) Award and Final Contract Values (3) Original and Final Completion Dates (4) Description of Scope (highlighting aspects of work considered relevant) (5) Telephone and facsimile numbers for points of contact (6) Additional information relating to the contract b. Project Management  The proposals shall address all key personnel. The offeror shall provide the resumes of the proposed project manager and project superintendent indicating the qualifications and degree of experience (minimum of 5 years) of these ind ividuals. The offeror shall identify if these individuals were assigned to any projects listed under Relevant Experience. Points of contact shall also be included in order to verify personnel qualifications. Provide an organizational chart that identif ies key construction and management personnel indicating lines of authority and dedication to this project. c. Critical Path Method (CPM) Schedule  The offeror shall provide a Critical Path Method (CPM) schedule from contract award through construction completion. Identify critical path activities, milestones and duration. Schedules will be considered accept able if they meet the required completion date and have a logical sequence of work. Section M  Evaluation Factors for Award 7. Evaluation Factors a. Factor I and II are of equal importance. Factor I and II are significantly more important than Factor III. b. Factor I  Technical/Management (Subfactors I-III are of equal importance) (1) Subfactor I  Relevant Experience (2) Subfactor II  Project Management (3) Subfactor III  Critical Path Method (CPM) Schedule c. Factor II  Past Performance d. Factor III  Cost/Price
